Hodges' Model: Welcome to the QUAD: interoperability

Hodges' model is a conceptual framework to support reflection and critical thinking. Situated, the model can help integrate all disciplines (academic and professional). Amid news items, are posts that illustrate the scope and application of the model. A bibliography and A4 template are provided in the sidebar. Welcome to the QUAD ...

care graph portability

care graph portability

care graph portability

Of course, social graph portability and care graph portability do not bear comparison. Social graph portability is still contested as the media reveals. But 'care graph portability' is stretching it...

In this post I wish to point out the internal transactions (conceptual portability - facets - patterns) that are needed to achieve person centred and holistic care at the individual level. This can then be integrated. Throughout an individual episode of care, to recovery and staying well this 'care graph portability' can be said to be achieved through reflective practice that also engages the patient and carer(s) to facilitate self-care and health literacy. This is true portability in the care context.

Consultation on the proposed withdrawal of the SNOMED CT national care planning content

In addition to responding myself, I notice on the actual consultation page on 'Citizen Space' there is an option to share this consultation.

Dear Peter,

You are registered for the care planning content via the HSCIC Technology Reference data Update Distribution website.

The national Care Planning Content is designed for use to support care planning functionality across health and social care. This functionality crosses many professions/sectors which may have differing descriptions for a care plan in the traditional health record (e.g. treatment plans, birthing plans, support plans, intervention plans or care order sets). The content can support the interoperation of care plans across boundaries and paper based care plans based on the pack have also been used successfully to gain greater familiarity with structured terminology (SNOMED CT).

More details at: ...

The absence of a declared owner or sponsor for this product means that we are proposing to withdraw it and are keen to hear views and understand the impact of not maintaining this content and its associated services.

We have a number of questions to determine how information Standards could best support the development of a standard for care planning, the consultation can be found at: ...

- and we would value and encourage your feedback on this proposal.

If you have any questions regarding the above you can contact us by sending an e-mail to information.standards AT hscic.gov.uk. This is the preferred form of communication.

BBC Horizon II: Processes - step this way ... safely

Following the BBC TV Horizon programme last night "How to Avoid Mistakes in Surgery" I came across the following list concerned with processes:

Sequential steps. Where one step follows another.

Alternative steps. Where conditional logic specifies which of several paths should be traversed.

Iterative steps. Where a single step is repeated multiple times.

Composite steps. Where steps are nested within other steps.

Optional steps. Where any one of a set of steps may be traversed, depending on the results of a boolean operation.

Checklist. Where all of a number of steps must be traversed, but in any order. p.126.
The list is related to software development and manufacturing as per the reference source below. Within informatics and project management I've noticed and written here about the emphasis placed on processes. Process, process and yet more - in one form or other. As A&E doctor Kevin Fong revealed people can get lost in a process (intubation), especially experts within organisational hierarchies - complex teams in urgent circumstances. In information system projects the need for a socio-technical perspective is essential, and is synonymous with due consideration of human factors. Processes are an essential ingredient to understanding the world and there's a huge literature on events, co-ordination, timing, logistics, value ... but as the Horizon programme pointed out there's also a need for reflection, stepping back, taking in or trying to grasp the big picture. Maintaining situation awareness is vital to assure safety and as the program showed in a crisis hindsight makes a blind spot visible. This is why process is one of four P's in Hodges model (the others - purpose, practice and policy). Processes count, but they are only part of the bigger picture.

The steps above are intended for software developers, but the descriptions throw up some points when subject to a more literal - human - interpretation (which might be a problem?).

The 'alternative' steps: could that mean skipping one in turn as in 'alternate'?

And crucially for the checklist are there any risks associated with its being completed in any order?

There is a world of difference between programming methods and logic on the one hand; and human language and dialogue on the other.

IHE-Europe Vienna Workshop
22-23 April 2009

Sharing clinical documents and integrating workflow

The event will provide an in depth review and discussion of IHE solutions in parallel sessions for users and suppliers. Also an opportunity to visit the 2009 Connectathon.

The Integrating the Healthcare Enterprise organisation (IHE) in Europe has asked IHE-Austria to organise this two day workshop. It will provide an in depth review and discussion of IHE solutions in parallel sessions for users and suppliers. The main focus will be on the nature and means of applying the IHE document and image sharing facilities to improve the real world of clinical practice.

There will also be the opportunity to visit the IHE-Europe connectathon, which will be held in Vienna at the same time. During the week around 80 different suppliers will show that their software can interoperate with that of three other suppliers to perform clinical tasks correctly as described in the relevant IHE specifications. Connectathon visitors gain a real understanding of the way IHE enables suppliers to work together to obtain interoperability of systems in a very enjoyable atmosphere. Experts from all healthcare specialties will be present including medical imaging.

For detailed information and registration visit the IHE-Austria homepage.
