Careif report: The Health Needs of the Indo-Caribbean Community in the United Kingdom

'The Indo-Caribbean community has played a key part in the life of the United Kingdom, but this has not often been recognised. Little is known about this populations’ physical and mental health needs and its experience of health and social services in the UK.

This report, The Health Needs of the Indo-Caribbean Community in the United Kingdom, examines the history of the Indo-Caribbean community in the UK, the prevalence of mental health conditions among the community, barriers to accessing mental health services, and the existing services available.'

Includes:
"The absence of a unique category designating ‘Indo-Caribbean’ as a unique category in the UK Census was identified as a major barrier to obtaining vital epidemiological data on the UK’s Indo-Caribbean population."

Above text:
https://www.careif.org/news-and-events/2025-10-9-careif-report-the-health-needs-of-the-indo-caribbean-community-in-the-united-kingdom

n.b.
In my community mental health nursing career in Chorley, Preston, and West Lancashire as services were established, the diversity of the local populations was evident, but there was an assumption that many communities were essentially 'closed': one explanation for lack of referrals from specific racialised communities - was that some communities 'looked-after their own'.

Of course this can be racism (acute or subtle), an excuse for inaction, delayed response, lack of action - outreach and translation services. There were marked examples of intervention. I have seen first-hnad interpreting and translation services in action since full-time retirement and the services offered are amazing for the insights and care delivery they can facilitate. Is this why we also need mental health nurses and the team? To translate in challenging circumstances, even by being there, seeking avenues, giving time - especially in instances of psychoses and doing so safely (for all)?

In addition to the main body of Careif's report and its recommendations, it may be helpful to consider the report's key terms relating them to the domains of Hodges' model? I also note reference to the bio-medical model as an explanatory framework. A conference abstract has been submitted today for April 2026, highlighting the need for bio-psycho-socio-political model / framework (Hodges' model - of course?).

"In traditional Bolivian dress, on the outskirts of El Alto, the Flying Cholitas launch themselves at each other. At first, I was fascinated by the skill of these indigenous women flying through the air, reclaiming the clothing their community was forced to wear as servants for Spanish occupiers. But then I saw it went beyond the ring. They are fighting for their rights, for recognition, for equality. They're fighting for their lives." Luisa Dörr

This image, would also 'sit' in the POLITICAL domain, if the women would stay still; but they clearly have individual and collective purpose.

"DG recognize that narrow identities form through social interactions and that groups play an important role in this process. This is a major advance. Because narrow identities are an equilibrium phenomenon they cannot be undone through individual will and right thinking alone. Specifically, DG’s model consists of a finite population of individuals, N, and two groups, A and B. Individuals and groups are ex ante identical. Individuals can choose to join one or both of the groups. Only the extensive margin (membership) is considered, not the time or effort devoted to each group. The payoff from joining group k ∈{A, B} is increasing in the size of group k and is also a function of the size of the other group k′ ≠ k. An individual i is said to have a narrow identity if i joins only one of the groups. Society can be said to have narrow identities when each individual i ∈ N joins exactly one group. Note that narrow identities can exist in a monomorphic equilibrium (where all individuals join one group) or a polymorphic equilibrium (where different individuals join different groups)." p.79.

Caleb's Crossing ... "Harvard's native son"

Mr Stephen Coit, the artist who produced the portrait above, also emailed ...

"Here are some notes on the painting that you might find interesting:

-- For Caleb's face, I used images of the earliest photographs I could find -- well over 100 years old -- of young men Caleb's age who were members of the Wampanoag Aquinnah on the Vineyard.  These would have been taken before intermarriage happened with any frequency with other groups.  While I was visiting their headquarters in Martha's Vineyard, the tribe elders uncovered and gave me permission to use these images for this project.  There are no extant images of Caleb's actual appearance anywhere that I could find at Harvard or anywhere else.

-- I planned for this to appear as if Caleb's portrait had been commissioned the year of his graduation, 1665.  In that era, many early colonial era leaders likely went to Europe to have portraits done, and they were often painted in a Dutch style after Rembrandt.  See John Eliot's portrait painted in 1659, (https://en.wikipedia.org/wiki/John_Eliot_(missionary).)  Note that Rembrandt died four years after Caleb graduated.

-- Seeking a Dutch painter of that era to inspire Caleb's portrait, I noticed that Girl with Pearl Earring by Vermeer was done the same year as Caleb's graduation, 1665. (https://en.wikipedia.org/wiki/Girl_with_a_Pearl_Earring)  Because I love Vermeer and his work, I chose to portray Caleb's drawing on the style of Vermeer, specifically using the light, setting and composition of Vermeer's, The Milkmaid.  The table, the book, the Turkish tapestry over the table in the portrait all confirm to Dutch portraiture conventions.

-- The bag on the wall is a portrayal of "Caleb's bag" in Harvard's Peabody Museum (https://www.peabody.harvard.edu/node/495.)

-- The map on the wall, again drawing on the style of Vermeer, is a map of Martha's Vineyard from the late 1600's showing the region on that island where Caleb was from.

-- Caleb's dress, his hair style and his collar were as accurate for that era as I could determine.

-- The room is plausibly a room in the Indian College at Harvard with morning light coming in to Harvard Yard from the southeast.  I wondered whether the leaded window panes would be rectangular or diamond shaped; an architectural historian assured me they could have been either.

-- The one element that would not have been a Dutch convention was the quill in Caleb's hand.  For Caleb's quill, honoring his history and tradition, he and I chose a turkey quill."

My sincere thanks to Mr Coit for permission to use the above image.

Seeking Care & Global Cultural Neutrality in White Houses(?)

The email concerned a book
- | White Houses | -
 and posed a question.

Needless to say it caught my attention ...

Is White a Statement or a Neutral Surface for Expression?

This is a question I ask myself, but in respect of Hodges' model.

To what extent is Hodges' model neutral across cultures?

Remove the green, blue and red colours I adopted for the care domains  and what you have is a blank sheet; an empty conceptual space.

Is it empty?

Very quickly it can become filled with what the person, patient, client, group ... brings with them.

Hodges' model is a stage. Several stages.

It is a theater.

It is a challenge for the performer, as they take their turn. As they navigate the space, they seek to ensure, assure, obtain feedback that the other - listener(s) are with them; whether in agreement or not.
Hodges' model is an arena for personal, group reflection, debate, argument(ation) or as described these days - co-production.

How suitable is #h2cm as a tool to support global health objectives, such as, the Social Development Goals and the Social Determinants of Health (and their ongoing development)? For me, Hodges' model is not just a neutral surface, it is a neutral substrate for the many forms of knowledge needed to solve the problems of the 21st Century.

"Roy McFarlane’s second poetry collection, The Healing Next Time, is a timely and unparalleled book of interwoven sequences on institutional racism, deaths in custody and of a life story set against the ever-changing backdrop of Birmingham at the turn of the millennium. Here forms a potent and resolute narrative in lyrical and multidimensional poems which refuse to look the other way or accept the whitewashed version of events."

Paper: Understanding the micro and macro politics of health [Mapped to h2cm]

"Almost a decade after WHO Commission on Social Determinants of Health published its influential report (2008), health inequalities within and across countries remain high on the research agenda. Acknowledging the complexity of the issue, scholars increasingly stress the need for the development of a theoretical framework that will integrate the multiple factors involved in shaping health inequalities, from individual social positions and experiences to institutions (Beckfield et al., 2015; Krieger, 2012, 2011)." p.92.

noun: intersectionality; plural noun: intersectionalities
  1. the interconnected nature of social categorizations such as race, class, and gender as they apply to a given individual or group, regarded as creating overlapping and interdependent systems of discrimination or disadvantage.

"Intersectionality as an analytical tool of social stratification (Yuval-Davis, 2015) challenges the idea of a single, fixed social hierarchy. It perceives social positioning as a spot within a matrix of intersecting power axes (Crenshaw, 1992). Hence, there are no sociological categories (e.g. race, gender) that have an a priori greater significance in shaping individual experience." p.93.
