'New' citation: "Pratique en milieu de psychiatrie légale: proposition d’un modèle interdisciplinaire."

Practice in forensic psychiatry:
A proposed interdisciplinary model

Checking through emails I missed a notification from ResearchGate about an additional citation for Hodges' model:

Holmes, D. Perron, A. Jacob, J.D. Paradis-Gagné, É. & Gratton, S (2018). Pratique en milieu de psychiatrie légale: proposition d’un modèle interdisciplinaire. Recherche en soins infirmiers, (Practice in forensic psychiatry: A proposed interdisciplinary model). 134, 33-43. DOI: 10.3917/rsi.134.0033

This is very welcome, even though in December 2021, I am 'late to the party'. This it seems is how news of potential additions to the bibliography are bound to arrive, which is a bit frustrating. It would be marvellous to learn of a project much earlier - while the work is 'live'. Not to jump on-board as a co-author necessarily, but to share the dynamism of work-in-process.

I continue to take encouragement from the fact that researchers appear able to find, understand and apply Hodges' model in their respective theoretical, practical and work context.

This is rewarding in itself so many thanks to the authors.

If I can access the English copy I will post again with any observations and comments that arise.

Translated by Google:

"Hodges' Health Career Model

This model has a politico-biopsychosocial structure which is consistent with contemporary interdisciplinary practice (20); that is, it relies on a multidimensional critical approach, incorporating writings in sociology and politics, in order to understand the person in context. It is based on four objectives: measuring learning, providing holistic care, supporting reflective practice and closing the gap between theory and practice (21). This model is applicable in various clinical situations in a psycho-legal context. When this model is used as a frame of reference, it emphasizes the role of caregivers who must meet the patient's needs and focus on their problems. It also serves as a guide to assess and provide assistance to the patient vis-à-vis their physical, psychological and social needs as well as with the justice system in order to promote their recovery. The theoretical foundations call on four sources of knowledge: interpersonal, scientific, sociological and political (21)."

In comparing several models of care please note how the authors here recognise how Hodges' model is not just bio-psycho-social; yes - it is politico-biopsychosocial.

The harmony of opposites: to relativize the continuous / discontinuous dualism by Michel Lussault

"If we accept a somewhat schematic presentation, we can estimate that the modern Western knowledge system has been deployed on the basis of opposition couples conceived as expressing unquestionable dualities. These couples organized thought, all its activities, under all its regimes including the knowledge of expertise and the knowledge of common sense, and they constituted a kind of cognitive dogmatics; That is to say, they made it possible to define the very relevance of the problems that could be posed. From these dualities, and for a long time from these dualities alone, it has been permitted and legitimate to pose a number of social, political and cultural problems and then to find ways of solving these problems. In view of our integration into the cultural space of the modern West, we all know and experience the prevalence of these antagonistic duos which have structured the thought and continue, albeit in a somewhat more discreet way, -tender." *

Metamorphosis (Ste p e-learning [curve?] )

I'm walking over ground of a gradually steepening pitch. At a certain point, I pause and start using my hands; the real mountain begins. I am climbing. Do I, as soon as my back slopes forward, return to the state of the quadruped? Almost: my body transforms; feet become hands and my two manual grips secure balance. Homo erectus, the standing man, of recent date, reverts back to the one from whom he is descended: the archaic quadrumane. This thunderbolt recollection became so black, in me, that I no longer fear to speak of the beast; I remember who we were (p.3).

As recent posts reveal I'm overwhelmed with reading at present. Material that is essential for the TEL course and reading more tangential and yet potentially enjoyable titles. It will be a busy summer as all this is brought together(!?). Especially as I've a literature search in mind as a module submission - progress permitting.

The publication of Jamison's book this (N) spring sits well after the findings last year about the benefits of reading fiction in increasing empathy.

Since 2008-2009 I've lost touch with the work of Michel Serres:

Jones, Peter, Exploring Serres’ Atlas, Hodges’ Knowledge Domains and the Fusion of Informatics and Cultural Horizons (Aug 15, 2007). SOCIAL INFORMATION TECHNOLOGY CONNECTING SOCIETY AND CULTURAL ISSUES. Available at SSRN: http://ssrn.com/abstract=1842504 or http://dx.doi.org/10.2139/ssrn.1842504

I only scratched the surface of Serres' oeuvre in the above paper and when time permits will revisit this work having just learnt of the Institute:

The resource-systems approach

"Reframing the conceptual and operational field of natural resources requires deep interdisciplinarity across fields encompassing legal and socio-economic studies, and life-sciences. And, it goes without saying, philosophy. The priorities at present are the aid to decision-making and a participative, pro-active civil society acting to (1) manage human activities while meeting the specific territorial potential and resource capacities and (2) ensure a fair re-allocation of such resources according to vital human needs and coherent public goods policies. Playing and working with words and music - étude : ctude

étude

An étude (/ ˈeɪtjuːd /; French pronunciation: [eˈtyd], a French word meaning study) is an instrumental musical composition, usually short, of considerable difficulty, and designed to provide practice material for perfecting a particular musical skill.

ctude

A ctude is a care assessment and evaluation instrument, a composition that is usually short, in summary form comprised of key concepts, and yet capable of representing the complexity of health and social care contexts. The ctude is designed to provide practice material for developing expertise in care assessment, holistic and integrated care that is also person-centred. Coined from étude (/ ˈeɪtjuːd /; French pronunciation: [eˈtyd], a French word meaning study) it is a study in care and can be a snapshot, or a dynamic series.

Abstract translation (100 words): Exploring several dimensions of local, global and glocal using the generic conceptual framework Hodges's model

I need to produce translations of an abstract into French, Spanish and Russian with a 100 word limit. At present I'm relying on Google translate; any suggestions to improve what follows (the English too!) greatly appreciated. 'Glocal' should prove a bit of a test. (h2cmng at yahoo.co.uk):

Exploring several dimensions of local, global and glocal using the generic conceptual framework Hodges's model

Abstract
This paper introduces Hodges’s model a conceptual framework as a means to explore the  concept glocal and the more familiar terms local and global. Actual and speculative definitions of glocal are offered. Discussion will also deliberate on the compound meanings of these terms. The model's four knowledge (care) domains facilitate discussion of the physical, social, political and individual dimensions of local, global and glocal. The paper draws upon health, anthropology, history, science, informatics and geopolitics – especially the themes of globalization, literacy,  information technology and communication (voice). The purpose is exploratory with additional resort to philosophical reflection.

Nurses & Social Media - mise en scène

... health on the thrust stage

From: Wordsmith.org
1. The setting of a scene in a play, movie, etc.
2. The setting or background of an event.

ETYMOLOGY:
From French mise en scène, literally "put on stage".
The health care domains model provides a 2 x 2 stage.

It may be a conceptual stage, but it is nonetheless demanding. It demands we perform the translation from theory to practice and check constantly exactly where we are AND where we need to be.

Should we become so ill that hospital treatment is needed the dependency and the sense of vulnerability this provokes often makes us feel that we are 'performing'. Not only that but as a  patient we find our selves on a thrust stage. When illness strikes it seems we are surrounded by this audience of clinicians who can potentially see us from all angles and be privy to very personal information about us.

The demands cut both ways though. Modern health care IS collaborative. Stagecraft here then means there should be minimal furniture and scenery to hide behind. Strange then that in terms of personalised care background details really do matter.

So, everyone matters. Teamwork counts. We try to keep patients off the stage. Patients and carers as expert carewrights may already know their lines - they can teach us. Suddenly everyone is an actor. Suddenly all the players must pass by way of the vomitory and the responsibilities that passage demands. On this stage care also needs to be exercised as per the direction of the NMC.

The art and science of Serresian spin

To Michel Serres:
- the middle is a point of reflection, a point at which questions - the question - must be asked. In fulfilling the purposes of the health career model:

  • bridging the theory - practice gap;
  • facilitating holistic practice;
  • supporting (personal and group) reflection;
  • enabling curriculum development;
- the model's four domains are constantly (re-)visited in turn. Motion is constant. Conception - birth provides that initial impetus. Health status. Life - momentum. Centered on the person the movement is usually self-correcting, seeking balance. Health care. Questions and answers whether whole or in part follow, leaving a trail of care delivered and care planned. A record.

There is definite synergy between our use of the health career model, the dynamic quality and quantity of health and social care and Serres' description of the spinning top:
"The behaviour of the cone or the top is worth analysing. Throw this toy and describe, as Plato did, what happens. It is in movement, this is certain, yet it is stable. It even rests on its point or its pole, the more so as its movement is rapid. All children know this. But its rest is still more paradoxical. The top may move about, by translation, without ever losing its stability. To repeat, it can do so as long as it turns very quickly.

Even better, its axis may lean, take on an inclination, without putting the movement of the whole in too much danger. It may again rock, by nutation, oscillating around a mean location. This very ancient and quite childish machine is marvellously instructive.

First of all, it combines and the movements known and thinkable at the time: rotation, translation, fall, leaning and swaying. An integral model, additive, overcharged, yet simple. Second, and above all, it conjoins in a simple one-off experiment phenomena judged or presumed to be contradictory. It is in movement and rest, it turns and yet does not move, it rocks and is stable. The simplicity of a complexity, first and foremost, an additive machine; a synthesis of contradictions, beyond anything else. Now it may serve as a little model of the world, for a naive simple and local orrery. It quivers, at rest, it moves forward, turning, like the heavens, like the stars." p.28-29.

earth, wind, fire, water AND the birth of physics

To begin - the health career model is concerned with space, structures and knowledge (care domains) built around two axes, plus the 'subjects' and 'activities' of health.

How we define and (so) divide space and accord that space salience amid changing contexts is critical to theory, practice and management and the models we subsequently derive:

... The dichotomy does not cut, it defines, it surrounds the closure of a limit, it delineates a boundary. Within the space thus enclosed like meets like. Or rather, conversely, the specific convergance [convenance] or identity, the assembly of the analogous, delimits zones in the disorder which are distinguished from each other. The earth is separated from the waters, air divides from fire. ... p.28.

Midwifery Materials for Haiti - in French/Creole - NEEDED

My source as posted to the GANM and on HIFA 2015 lists:

I am writing the (GANM) community to ask for assistance in gathering online midwifery and nursing materials - in the French or Creole language, for Haiti.

The National Nurse School University and Faculty of Medicine in Haiti has collapsed and they have lost most of their training materials. Dr. Larouche was killed in that collapse - he was teaching an OB course to the midwifery class at the time. I do not know the fate of the nursing/midwifery students, but one can imagine that the outcome is probably not a good one. Dr. Larouche was filling the void left by the untimely death of Madame G.Francoeur last year (she died in an auto accident).

Midwifery in Haiti was just recovering after the sadness of the loss of Mme Francoeur - and the earthquake has now destroyed most everything that they had.

Agnes Jacobs, UNFPA survived and has been emailing from Haiti. She and her colleagues are working to pick up the pieces and try to reconstruct materials for training midwives and nurses in life saving skills.

I am writing to members of the GANM asking for support in finding DIGITAL materials that we can provide to Agnes for the time being. It is VERY hard to get shipments of books and things to Haiti right now, but in the future, I suspect that Agnes will be able to use books, and guides, and computers to rebuild midwifery in Haiti. So eventually we will ask for books and things, but right now it is very hard to get these types of things to her. I know she is not alone, I know that COHI, PAHO, and many others are on the ground. I think the GANM can work with all these others to help in whatever way we can.

Johns Hopkins is sending a team to Haiti, including Dr. Beth Sloand who has been travelling to Haiti with students for over a decade. Beth does not have room to take printed materials, but she can maybe take a packed hard drive or CDs/DVDs that we load with teaching materials. Agnes does have a computer and internet (sporadically) - so I have been discussing with her the potential of sending digital materials which she can start to use right away until printed materials can be provided.

She welcomes health materials in French and or Creole primarily. They should be materials that can *instantly be used to teach midwives and community health workers* - so scientific articles that require interpretation and reconstruction into teaching materials are not what I am asking for at the moment. Right now there are no resources to turn scientific literature into teaching materials. Do you have teaching materials that you would be willing to share?

I would like to ask that people send either direct links to online resources that we can copy down to a CD or send useful teaching materials that we can put on a CD.

Please email them to the GANM or to me (pabbott2 @ son.jhmi.edu)

Regine Marton wrote already directing us to look at Hesperian and the
WHO Reproductive Library (in French). We have the Hesperian material now. The WHO library is helpful.

If you have any quality materials for training of midwives that you would like to donate or direct us to find, please let us know. Please send scientifically sound materials - the last thing that Haiti needs are questionable methods or what we call in the US "hocus-pocus" (unproven claims or "magic").

I hope this makes sense and that the GANM community will come together to help Haiti in a “different” way. I think what we are doing is unique – while of course there are huge needs for emergency care and the like we are looking one step ahead to helping Haiti rebuild its educational infrastructure.

Health care and handedness

The word legerdomain featured on Wordsmith's Word.A.Day as listed below:

PRONUNCIATION:
(lej-uhr-duh-MAYN)

MEANING:
noun:
1. Sleight of hand.
2. A display of skill.

ETYMOLOGY:
From French leger de main (light of hand), from leger (light) + de (of) + main (hand). Ultimately the from Indo-European root man- (hand) that's also the source of manage, maintain, maneuver, manufacture, manuscript, and command.
It is a shame and a matter debated at large that specialists - whether medical or of other disciplines - are not held in the high esteem that they once were. And yet when you come across a leader and specialist in their field the way they deploy their knowledge, insight and skills remains something to greatly admire and respect. I mean knowledge and experience that transcends sole reliance on interpersonal skills and blinding people with jargon and 'science'. Such are the outcomes of what the specialist brings to the (operating?) table, that it may even seem like a sleight of hand. Not in the sense that it is a trick, but there is an effortless transparency to not only what they achieve, but how they make people - their patients, clients feel. Not just feel, but how these people react and may find themselves.

If there is a sleight of hand it is in how while a specialist, they can and do cross boundaries. They see and can count the important numbers (evidence) in their discipline, but they know (from some no doubt hard-won lessons) that frequently some of the numbers in the very sequence they need lie outside their comfort zone.

RIGHT-handed (mechanistic)
or
(humanistic) LEFT-handed

- they know there is a
middle
and they cross it, without anyone noticing.
