Save Cardiff University School of Nursing - Sign the Petition

Cardiff University have announced that the School of Nursing is set to close with cuts to staffing because of a funding shortfall.

We condemn this action and urge the university to find another solution that does not involve closing the School of Nursing.

This awful decision will have a direct impact on the number of nurses available to work in health boards across the country . At a time where there is a significant number of nursing vacancies within the NHS this action will undermine an already depleted workforce.

With well over a 1,000 students within the department, Cardiff University School of Nursing contributes a very important number of nurses which are employed within the NHS each year.

The school of nursing is ranked 1st in Wales and is an centre of excellence. It is a dynamic, innovative and forward looking school. They are recognised for their excellence in learning, teaching and research. They pride themselves on pursuing research of the highest quality, and are one of the leading healthcare departments in the UK.

As well as this, the staff  work incredibly hard, have invaluable diverse expertise and put their hearts and souls into making the school a vibrant and welcoming place. The nursing department is full of warm, intelligent, passionate people with a wealth of knowledge, who are at danger of losing their livelihoods. We stand in solidarity with the university staff affected by this decision.

Please sign this petition in the hope that Cardiff University reconsider its decision to close the School of Nursing.

National Survey of the Mental Health Nurses aims to understand reasons behind record vacancies

Researchers from the University of Southampton working with the National Institute for Health and Care Research (NIHR) are launching a national survey of the mental health nursing workforce.

Led by Professor Jane Ball (Professor of nursing workforce policy), it is the first to explore the work lives and wellbeing of registered nurses providing mental healthcare in the UK.

More details:

The survey is open to nurses on the NMC register providing mental healthcare to any patient group, in any setting, and for any health and social care provider. It is completely anonymous and will take 15-20mins to complete. https://tinyurl.com/MHNurses

"Nurses' ability to reflect on their practice is vital to the profession" Is it?

I beg to differ with the title of this editorial, as I wonder how vital reflection really is; and not just to nursing, but to the profession. I wonder from the rather detached perspectives of:

  • Wigan Pier - and the parochial view this engenders (#globalhealth - where on Earth do I think I am?);
  • As a 'local' nurse practising in Bolton;
  • As a mental health nurse practising in the community within the NHS
  • As a champion of a pivotal* resource for reflection and critical thinking (where is everyone?);
  • and several others ...
Nursing Management
Quinn notes how "Nursing continues to develop to meet the needs of the society in which it operates." Plus: "A core part of revalidation is the ability to reflect on, and learn from, our interactions with others." My reading of Kinchin (and others over the years) suggests that reflection needs to be more a practice and revalidation based. I understand this is not what Quinn is saying in this issue on "Reflection as part of revalidation". Reflection using Hodges' model can be preemptive and anticipatory. We need to get beyond introspection and the retrospective important as this is.

As to where people are, it seems they are elsewhere, maybe reflecting in the sciences domain seeking theories and evidence. Perhaps, current teaching and exposure to models and theories of nursing and healthcare has them reaching for 'pure' models of reflection - Boud, Gibbs et al. when necessary. As we talk about the legacy issue that is integrated care, in Hodges' model here is a tool - a 'freebie', a 'takeaway' yes an Ubercare, that can grow with students, newly qualified nurses and experienced staff as lifelong learners.

Now that is vital to me. When reflection is hard-wired**, a part of a practitioner's cognitive apparatus and their educational toolkit to share with the public explicitly or implicitly as the context and engagement demands.

*Pivotal? Yes, and of course in light of the above, I'd be pleased to explain and even argue the case. The irony here is the current (ongoing!) focus upon recruitment and retention. Hodges' model is 'just' a tool, I recognise this; but could the application of Hodges' model energise, inoculate and so prime students for their course of education and practice placements?

**soft-wired too - humanistic

The New Script of Nursing : El Nuevo Guión de Enfermería

Dear Colleagues and Members of the Global Alliance for Nursing & Midwifery (GANM),

All of us know that our profession is undergoing an extraordinary transformation and that nursing will never look the same. While we know this, there is a profound need to educate the general public about the broader scope of our work and expanding opportunities.

As the breadth and impact of our work increases, so does the need to recruit and retain nurses—crucial elements during a nursing shortage throughout the world. Together, we are writing the “new script of nursing,” and we have created a video that reveals the intensity and magnitude of our profession that is much more than what meets the eye – researcher, clinician, change agent, inventor.

We purposely did not brand this video as our own because the advancement of the profession and the healthcare needs of the patients we serve are more important than any of our individual or organizational needs. By not branding, the message will go farther and have greater impact.

I encourage you to use it as your own and help spread the word of the new script of nursing. Like many of you, we can’t support a national campaign, but together we can perhaps start the process of educating the public and moving nursing toward a realistic, contemporary image.

Share it on social media, send it to your colleagues to push out, and join us in saying “#WeGotThis.”

Patricia Davidson
Dean, Johns Hopkins School of Nursing

--------------------------------

Surveys, Nursing Education, Drupal and Webform

Just before heading to Drupalcon Prague I learned of a new paper by Gill et al.:

Using a web-based survey tool to undertake a Delphi study: Application for nurse education research

Background: The Internet is increasingly being used as a data collection medium to access research participants. This paper reports on the experience and value of using web-survey software to conduct an eDelphi study to develop Australian critical care course graduate practice standards.
Methods: The eDelphi technique used involved the iterative process of administering three rounds of surveys to a national expert panel. The survey was developed online using SurveyMonkey. Panel members responded to statements using one rating scale for round one and two scales for rounds two and three. Text boxes for panel comments were provided.
Collecting data and providing feedback: For each round, the SurveyMonkey's email tool was used to distribute an individualized email invitation containing the survey web link. The distribution of panel responses, individual responses and a summary of comments were emailed to panel members. Stacked bar charts representing the distribution of responses were generated using the SurveyMonkey software. Panel response rates remained greater than 85% over all rounds.
(in press)
The paper is interesting in combining surveys and an e-variant of the Delphi research method. Conclusions highlight the improved accessibility to experts and how ethics needs to keep pace with technical change and affordances.

Yesterday at Drupalcon there was a presentation on Webform:

Webform 4.0: Surveys in Drupal Improved

I've mocked up a enquiry form using Webform, but this session pointed to how the module can be extended for polls and contact forms. Based on Gill et al. and the eDelphi technique they explain this could be a really powerful combination.

The Willis Commission on Nursing Education: Recommendations

Here are the recommendations from this commission (the full report is also available):

  • Patient centred care should be at the heart of all pre-registration nursing education and continuing professional development.
  • There were no shortcomings found in nursing education that could be directly responsible for poor standards of care or a decline in care standards.
  • Nurses and their organisations must stand up to be counted on the challenge of poor care and loss of public confidence in order to restore professional pride.
  • Nursing education needs to imbed patient safety and dignity as a top priority.
  • Better evaluation of and research into nursing education programmes is necessary to ensure a programme that is fit for purpose.
  • The future nursing workforce requires nurses to work in a variety of settings.
  • Recruitment campaigns need to widen their diversity in order to encourage the widest, best possible range of applicants.
  • Health care service providers must fully support nursing education.
  • Universities need to recognise nursing as a practice and research discipline.
  • Attention needs to be paid to developing a strategic understanding of the nursing workforce as a whole and as a UK-wide resource.

    Nursing: magnetic Force 5

    Back in 2009 I came across a post - Nurse magnets crucial for recruitment and retention about the 14 Forces of Magnetism:

    In 1983, the American Academy of Nursing conducted a survey of 163 hospitals to learn why some hospitals attracted and retained well-qualified nurses who were devoted to quality patient care.
    The 14 Forces are listed and described by the ANCC. The forces themselves include:
    • Force 1 Quality of Nursing Leadership
    • Force 2 Organizational Structure
    • Force 3 Management Style
    • Force 4 Personnel Policies and Programs
    • Force 5 Professional Models of Care
    • Force 6 Quality of Care
    • Force 7 Quality Improvement
    • Force 8 Consultation and Resources
    • Force 9 Autonomy
    • Force 10 Community and the Hospital
    • Force 11 Nurses as Teachers
    • Force 12 Image of Nursing
    • Force 13 Interdisciplinary Relationships
    • Force 14 Professional Development

    The professional, organizational, and political (policy) emphasis of the 14 forces is obvious and becomes clear when each is weighed in terms of where it sits within the domains of h2cm.

    Try it as an exercise. ...

    Recruitment is ALL about magnetism.

    If you are unsure, ask a magnet about the meaning of retention.

    Demographics are already applying pressure upon these forces of magnetism. Not just when referred to explicitly in the USA within organizational media; but globally. Demographics is another magnet - it approaches with increasing force.

    From here in the UK (and readers elsewhere) we have to exercise care when models are mentioned. While the theorists and philosopher's of nursing nail their definitions to the mast (h2cm?) there remains a models muddle, not just in the variety of models of care, but in the levels at which they operate. This is not a criticism, it's an observation - consider Force 5:
    Force 5: Professional Models of Care
    There are models of care that give nurses the responsibility and authority for the provision of direct patient care. Nurses are accountable for their own practice as well as the coordination of care. The models of care (i.e., primary nursing, case management, family-centered, district, and holistic) provide for the continuity of care across the continuum. The models take into consideration patients’ unique needs and provide skilled nurses and adequate resources to accomplish desired outcomes.
    In the US in particular 'models of care' (moc) often refer to finance and accountability of costs (the market process), in the UK moc might refer to commissioning. In Force 5 the addition of 'Professional' (as the original author's no doubt recognized) is crucial. If you repeat the above exercise, plotting Force 5 on the Health Care Domains Model then you see how Force 5 works for nursing and remains to this day a great achievement as a yardstick for quality, assurance and retentive power.

    In the almost 30 years since the research on the 14 Forces, I do wonder though if there is a need to imbue the following with magnetic properties:
    • person-centred care;
    • self-care;
    • carers and public engagement;
    • prevention;
    • public (mental) health
    • and informatics?
    Yes, many of the above can be assumed to lie within the existing Forces 1-14. Health and social care are not static. Nursing has much to contend with from the level of the individual practitioner through to the group within an organization. The 14 Forces of Magnetism are well established in the USA and deservedly so, they clearly deliver.

    In the political and economical heat of an economic recession, however; magnets may reach their particular* Curie point. Then they cease to work.

    The constant bangs and knocks of change, the incessant hammering of party politics and the 'market' on the door of "high quality nursing care" can also take its toll on magnetism.

    Nursing needs to take care.

    Related post on Healthcare IT News:

    Top 10 trends for 2011 include IT, new care models

    *OK it should be constant, but like our patients these magnets are not all the same - they have varying levels of vulnerability.

    To follow some definitions from an olde book.
