Revisiting the G7 ... G20

'Under the Trump administration, the US is retreating from the global leadership role it has played since the end of the second world war. The world is bedevilled with challenges and there is a desperate need for stability and co-ordinated solutions to global problems. Where will the leadership to solve them come from?

Some have argued that we are now in a "G zero" world in which no one is in charge. There is great uncertainty and the powerful dictate solutions to smaller players. Others say we are moving to a regional carve up - a G3 where the US. Russia and China have spheres of influence in a modern day version of the Three Emperors League.
Some version of a new multi-polar order may yet take shape. But it also seems clear that a G minus one world is starting to fill the vacuum in some domains — with countries coming together to find solutions to global problems without the US.' p.11.

Social Care: in the round and square c/o King's Fund

Simon Bottery's Long Read -

What’s your problem, social care? The eight key areas for reform

- and listed below, have been mapped to Hodges' model. The full article and comments are well worth reading and may help readers discern the relationships between the key areas identified.
  1. Means testing: it’s not like the NHS
  2. Catastrophic costs: selling homes to pay for care
  3. Unmet need: people going without the care and support they need
  4. Quality of care: 15-minute care visits and neglect
  5. Workforce pay and conditions: underpaid, overworked staff
  6. Market fragility: care home companies going out of business
  7. Disjointed care: delayed transfers of care and lack of integration with health
  8. The postcode lottery: unwarranted variation in access and performance

I have associated each with the respective knowledge (care) domain in which they are placed:

1. Parity of Esteem in Means Testing
2. (Catastrophic) Cost to me, anxiety, depression, stress
3. Unmet need (mental health, counselling ...)
4. Quality of Care (Mental Health, Well-Being..)
5. Workforce (values, person-centred, self-esteem, job-well-done, BE the Difference?)
6. Market fragility (attractiveness as career pathway, having a leader)
7. Disjointed care (number of carers, lived experience...)
8. Postcode (I thought this had been addressed?)
(postcode? - a remote 'thing' outside through the window and (summer) garden]

1. Parity of Esteem in Means Testing
2. (Catastrophic) Cost to NHS - innovation, change, prevention, transformation
3. Unmet need (physical, access ...)
4. Quality of Care (Time, Logistics)
5. Workforce (tasks, scheduling, physically reasonable?)
6. Market fragility (scale, bed occupancy, local demographics...)
7. Disjointed care (geography, transfers, A&E admissions...)
8. Postcode ('literally') North, West, East, South,

3. Unmet need (socialising, access, carer ..)
4. Quality of Care (Relationships, Belonging, Communication, Family Groups..)
5. Workforce (job satisfaction, status, social worth ..)
6. Market fragility (Public perception, marketing, negativity bias - local news, time for outreach, intergenerational engagement...)
7. Disjointed care (input of family, local provisions, self-funding - budgets...)
8. Postcode (pre-social care, pre-lottery?)

Patient-centred + Treatment = Self Care Week

This week 18-24th November is Self Care Week

http://www.scie.eu/scwe/

Hodges' model might tend to have us equate 'self care' with the humanistic half of the model. As Self Care Week begins I could not help but suggest on twitter that what self care means to me is 4-fold care (or 5-fold taking in the spiritual also).

Then, continuing to read Kinchin more on threshold concepts, care and caring:
"In the case of 'caring' within the clinical sciences (e.g. Clouder, 2005), the concept may link the salient points of the personal perspective (patient-centred discourse) with the biomedical (treatment) discourse (Figure 32).

Here 'care' is seen to occupy the space that links caring as a therapeutic intervention (to the left) and caring as the nurse-patient interpersonal relationship (to the right), as described by Morse et al. (1990). This positioning enables the carer and the patient to be active partners in linking the chains of clinical practice with the networks of understanding that relate to the patient's wider needs. The key factor within this model is the 'care' that includes consultation with the patient and carer that allows them to relate the two halves of the model - something that is required for learner agency (Kinchin & Wilkinson, 2016)." p.90.

The halves referred to in Figure 32 are those within Hodges' model, merely switched left-right. I will add a photograph soon.

Self care must encompass ALL of Hodges' model.

Yes: to occupation, earning or volunteering

Apologies in advance, as the formatting may be disrupted on some devices so two lines may not scan across the care domains as intended.

As you can see this brief 'sketch' of #StillMyLife shows that the emphasis of this planning tool is well-placed. This also highlights what I have referred to as 'holistic bandwidth'. There are many claims regarding the centredness of various things, for example; student education and learning, health and nursing care, government and the way business is conducted. In this instance, the question of what is the conceptual, disciplinary and professional extent of a tool, approach, theory or practice. Is its claimed philosophical, ethical, social or other objective fully deserved? Is it person-centred, or task centred? Does it deal with the political dimensions of significance to an individual with a disabling health condition as it claims?

POPULATION

You would like some soup.

Yes.
We should start here.
[For as the ancients recognised:
"In-between MIND and BODY lies the soup."]

As we can (and do) debate exactly 'where' and 'when' action begins.

Do you know you're hungry?
Do you know you want soup?
Do you know where it is?
Did you buy some three days ago?

If you don't, do know how to make this happen?
Can you ask someone for help if needed?
Do you know where the kitchen is?
Is your kitchen, one that you can walk in
(also known as your's),
or is it one of those "off-limits" designs
(see below right)?

Like many of the things we take for granted
Soup IS Knowledge

There you are! In the kitchen.
You are aok opening the can, carton.
Home-made excellent!
[And the bread! Wo!]

Can you reach for and open things?
Grip, twisting it is quite an operation -
fingers, hands, wrists, balance.

We don't want you to spin around do we?
Most have a opener of some sort,
maybe a drawer or a rack full.
Perhaps something to sit - perch on would help?

Can you read the instructions?
tiny print and poor eyesight

What about Imperial and Metric?

Take care with that can?
Safety first, cooker, gas...?
Are you up to date with Tetanus jabs?

Where's the plasters?
1st Aid Kit.
"Is there a Dr in the house! :-/ "
Quick - grab the ice!

How does the soup taste?
Taste?
Yes it is  subjective ... but...

Are you on a big hike?
If so, it's the best soup you ever had?
Attitudes to food inc. soup are laid here.
Somehow it tastes better in company.
Like its constituents soup is best crowdsourced.

Here is the place
and down the agees
 that the crowd (family-friends)
together with home-made
are ingredients made for each other.

Ah! They're here.
My Carer = company + soup ...

How much time don't we spend on a communal meal. An event in itself.

Thanks Stan your soup was delicious!

Whether a kitchen,
outside oven,
or camp fire
this place is a powerful place.

Assessment for Personal Independence Payments

Powerful too are foodbanks and soup kitchens.

Oh dear.
Yet another new carer = lost time
'dear' indeed.

Stop!
Are those vegetables ok?
What's the BBE - Expiry?
So much food goes to waste - tragic.
Is there an Occupational Therapist in the team? What about a Physio, or do we have to refer on?

Who controls the labelling
on this product anyway?

1 in 3 will be older adults by 2025 in Japan

In June Dr Mayumi Hayashi described Japan's vision of 'total care' for its older population in HSJ. The article that prompts this post follows another with lessons for England.

Referring to a "2025 vision" this forward thinking has its roots in established systems of healthcare set up in 1961 and social care established in 2000 (p.25).

Care integration is not new as a fundamental issue in health and social care. It is for me a career legacy issue. As a student nurse it was discussed and debated, closely allied with multidisciplinary and holistic (joined up physical and mental health) care. Even now 37 years later it will drive many arguments and policy deliberations in the run up to the next election here.

Many nations are faced with stark demographics. As the population ages and works its way through wooden blocks, Rubik cubes, it is the population pyramid that takes on increasing significance.

Dr Hayashi lists the need for inclusion, integration and continuation of four components that are essential to the realisation of this vision:
  • maximising the integration of healthcare and social care;
  • promoting policies for prevention and outreach together with safeguarding;
  • embedding supported living programmes and dementia friendly community initiatives; and
  • addressing “late life specific” housing needs.
I have mapped these to Hodges' model below:

individual

humanistic ------------------------------------------- mechanistic
SOCIOLOGY : POLITICAL
group
embedding supported living programmes and dementia friendly community initiatives integration of health and social care
"late life specific" housing needs
(integration of health and social care)
promoting policies for prevention and outreach, together with safeguarding

It becomes clear to see in Japan, China and other nations how telecare and smart homes have a role to play. Getting the basics of integrated care resolved firstly is the prerequisite whatever the culture.

Where achieved the integration of health and social care can act as a diagonal brace as it straddles two care domains. Perhaps the model also reflects the ongoing challenges of parity in esteem in mental health care and physical care; and the funding ambiguity for people living with dementia as opposed to other medical conditions?

In January 2014 the FT Weekend magazine also featured an article on ageing in Japan.

So many gardens: Press Release [ now ]

It had to happen. Twice over.

The irony that just as so many homes with older people living in them have gardens that are in a state of neglect, so too do many nursing and care homes.

Of course this is a sign in those domestics homes of reduced abilities.

In the residential care and nursing homes, garden neglect is reassuring.

A $ure sign that the love and spending is on the blooms inside?

A New Dynamic of Ageing: The NDA Programme & Age UK research showcase of the decade

London, The Business Design Centre, 52 Upper Street, Greater London N1 0QH

You’ll know that the UK population is ageing. You’ll know that there is significant work to be done to be better prepared for ageing in our society. So it’s crucial that all of us who work in and for later life use latest evidence on ageing to support those preparations. The New Dynamics of Ageing (NDA) Programme and Age UK are proud to provide a unique opportunity to present that evidence at our joint event, held on Monday 21 October, at the stylish Business Design Centre, Islington, London.

This is the ageing research showcase of the decade, presenting the most comprehensive selection of state-of-the-art ageing research ever seen in the UK. This includes active ageing, quality of life, independence, dignity, money, work, environments, participation and connectivity to name a few. Throughout the day, you can hear from high profile speakers, pick up latest knowledge through our themed sessions, interact with exhibits in our innovative marketplace and meet the researchers. It will also be your chance to network with a wide range of colleagues from the public, private and charity sectors.

Watch this caring space: Workshop - Robots 4 independence

The KT-EQUAL initiative has another event scheduled for later this month, another workshop on the application of technology to improve the quality of life of older people and their carers. Please find the call copied below. The workshop I attended in September and reported on here was very enjoyable and a great source of information and inspiration.

The topic for the 24th November paradoxically sends me back to The Which Computer Shows at the NEC in Birmingham which I used to attend with my father. In the late 80s and 90s the pace of change was clear to see. The topic for the 24th November is Robots and of course they are already everywhere, but the combination of:

  • demographic change - ageing population(s)
  • hence need to focus the skills and knowledge of the care workforce
  • emerging and refinement of safety and quality standards
  • design and innovation
  • the rise of 'single households'
  • mobile comms and ubiquitous computing
  • the expectations of the baby-boomer generation (and those who follow!)
  • lessons learned through early telecare(?)
  • and much more besides
- means that change isn't just on the cards, it's in neural nets, speech synthesis and the programme for this event. So as they say watch this space - it will change and move! Here is a marvellous opportunity to find out what is really happening in this field ....

Robots supporting personal independence and rehabilitation

This workshop organised with the University of Hertfordshire will present and discuss the role of technology in the rehabilitation of older people and children, including some of the latest developments in the use of 'robotic' technologies. The perspectives of research scientists, clinicians and users will be explored through presentations, videos and breakout sessions.
Hosted by: University of Hertfordshire
Sponsored by: KT-EQUAL
Event organisers:
Dr Farshid Amirabdollahian, University of Hertfordshire
Professor Kerstin Dautenhahn, University of Hertfordshire
Professor Gail Mountain, KT-EQUAL Director
Verity Smith KT-EQUAL Coordinator
Date: 24 November 2009

Venue: The Comet Hotel (Ramada Hatfield Hotel)
St. Albans Road West
Hatfield, AL10 9RH
Hertfordshire
United Kingdom
This workshop aims to stimulate debate among key stakeholders regarding the nature of assistive technologies developed using the latest developments in robotic technologies.
It will include presentations from experts working across the spectrum of development, from basic technology to neuroscience.
The workshop will be of interest to a wide range of practitioners and policy makers, health and social care practitioners, employers, charitable and government bodies concerned with the needs of older and disabled people, as well as researchers and academics from engineering, biological, social science, medical and health care disciplines especially those who are interested in looking forward to a time when radically new technologies will be available. Older people are especially welcome at this workshop.
There is no charge for attendance just an enthusiasm and interest in extending the quality of life through informed user-focused research and its application. Registration is essential. To book a place, please refer to the registration page.
