The Line(s)

Foresight, hindsight, sight itself and that illusory association with the line of time is a fascination for us all. No doubt, as a result of the hardwired analogue found in the lifeline and our ultimate fate. Hodges' model acknowledges all these lines; concrete and abstract. The work and health careers, an individual's education career (where accessible / permitted) now lifelong learning.

Abstract things seem tenuous, indistinct, maybe lost somewhere in theory. Concrete on the other hand, is physical, tangible.

Hodges' model can cover paper with lines, two of them ideally. The model can then be applied in practice.

There is then an interplay between abstract and concrete; between which data, information and knowledge flows, uncertainty reduced. Scope and scale become clearer once assessment, planning, intervention and evaluation are concurrent.

This applies to all lines, if they are not destined to be covered in sand.

RCN Congress 2025 ii - Losing direct nursing skills

No longer an issue for me, but in the mix of careers: health, education, work - professional; employees often find that they are pulled away from what originally brought them to their chosen career. "I want to help people, care for them, make them better." Especially, if they are ambitious, seek better pay and prospects, for themselves and (a) their family.

This was an issue and tension for discussion at the RCN Congress:

'Climbing the ladder often means leaving the “bedside”, this is something that can leave mixed emotions for the individual. As a result, we are losing years of clinical experience and expertise which could impact on the future training and on-the-job development for early career or inexperienced colleagues.

The secondary issue that arises with nurses leaving the bedside is skill decay, where a person loses the skills developed in the clinical area through lack of everyday practice. This is being increasingly acknowledged in health care literature (Maehle, 2017). However, the biggest barrier is the resulting lack of confidence in personal skills and ability, particularly where someone is now seen as a senior colleague by those at the bedside.' cont. ...

Discussion - Losing direct nursing skills

I didn't make notes but reflected on how your 'basic training' (for me ...) registered mental nurse (RMN) and state registered nurse (SRN) provides the foundation for professional (and lifelong?) learning? This is, after all, what continuing professional development and post-registration refer to. I did realise that in returning to mental health nursing after qualifying as a SRN, I would surrender much learning - instead of consolidating.

Lecture 101, sets in train the scope of your theory and practice. This is why, and where I see Hodges' model having a role at this point in our professional education, whatever the discipline, or profession. Congress included a resolution for debate: Physician associates.

We really do need to get this right. Get it wrong, and the quality of care and safety of the public are threatened. If existing standards and professional relationships are perturbed, the effects can be long-lasting and (grossly) injurious. Especially at a time of increasing demographic pressures globally for health systems and services.

2. But, wait! What does history, medical sociology have to say?

3. Is the 'bed' the signifier of instititionalised care? It appears to be.

The pejorative term 'big bins' seems to epitomise the mechanisation of care. In prisons do they throw away the key?

RCN Congress 2025 Liverpool i

... the twists and folds of debates

Repeating myself (again) but worthwhile I think(?), the past week brought my second RCN Congress. If you are new student nurse, international (Std) nurse, or nursing support worker and not engaged with a professional body, or union; do try to get involved. Don't leave it as long as I have (I joined a union as a nursing assistant). By way of a small excuse, listening this week it seems I am not the only one to leave it 'late'. Based in Liverpool (again after 2019) means there's a torrent of potential content. So where am I/We? By their nature all the debates are worthy. I have picked a few:

Discussion - Celebrating a nursing career

You can read above the main focus for the discussion. For me it prompted the following (no doubt - familiar)  reflections ...

The number of potential careers in health and social care is amazing:

Not only what the NHS (for training, education) can 'offer', but within that the number of nursing roles and specialities. The discussion inevitably drew the 'longevity' card. Students do pick on the temperature of their chosen community of practice. They recognise when they are surrounded (not literally!) by colleagues who have careers numbered in the several decades. But like a card the side that presents the occupational safe harbour from the threat posed by artificial intelligence, is countered by the seemingly relentless pressures imposed on nursing staff. The debates also revealed the pressure and negative impact of 'lived' work experiences for students and newly qualified nurses. I remember in the late 1970s early 1980s student nurses being counted as 'staff'. Have we really not progressed? I wrote 'offer' above because many students cannot get jobs once they have qualified. Not the best start to a shiny career. Having to fold, as they qualify.

Speaking of "career", can we please twist that? There is a great deal of talk - soul searching - about how on earth the (so-called) health service truly transforms itself to be health promoting and preventive. In the NHS's history perhaps there was a window of opportunity to begin in the late 1960s. Not so much a 'moonshot' as a healthshot. Now with the demographics that potential is being lost, unless this is positive way that AI can make up the shortfall? Alternately, nursing can utilise the health career, as in Hodges' model which are invariably impacted by an individual's life chances. As such there may be signs and signatures in the person's familial background? We can imagine a timeline composed of Hodges' model as 2x2 frames through a person's life, even prior to their conception and their legacy.

More to follow . . .

Previously: 'RCN' : 'nursing' : 'power'

Please pardon the 'gambling' analogies, but we are literally asking students to gamble with their careers, financial status and well-being (life chances - no less). Should they be lost to the service, you can't blame young people. They know the logic of care, also known as demographics. They can do the maths? So who is it who can't do basic arithmetic?

Save Cardiff University School of Nursing - Sign the Petition

Cardiff University have announced that the School of Nursing is set to close with cuts to staffing because of a funding shortfall.

We condemn this action and urge the university to find another solution that does not involve closing the School of Nursing.

This awful decision will have a direct impact on the number of nurses available to work in health boards across the country . At a time where there is a significant number of nursing vacancies within the NHS this action will undermine an already depleted workforce.

With well over a 1,000 students within the department, Cardiff University School of Nursing contributes a very important number of nurses which are employed within the NHS each year.

The school of nursing is ranked 1st in Wales and is an centre of excellence. It is a dynamic, innovative and forward looking school. They are recognised for their excellence in learning, teaching and research. They pride themselves on pursuing research of the highest quality, and are one of the leading healthcare departments in the UK.

As well as this, the staff  work incredibly hard, have invaluable diverse expertise and put their hearts and souls into making the school a vibrant and welcoming place. The nursing department is full of warm, intelligent, passionate people with a wealth of knowledge, who are at danger of losing their livelihoods. We stand in solidarity with the university staff affected by this decision.

Please sign this petition in the hope that Cardiff University reconsider its decision to close the School of Nursing.

Hello? Is anyone there? 'The Homeless Mind'

individuality

INTERPERSONAL : SCIENCES

SOCIOLOGY : POLITICAL

group

'Put differently, the componentiality of the cognitive style pertaining to technological production extends to identity. Again, a specific kind of double consciousness develops. In this case the dichotomy is between concrete identity and anonymous identity. The individual now becomes capable of experiencing himself in a double way: as a unique individual rich in concrete qualities and as an anonymous functionary. This dichotomization in the subjective experience of identity makes it possible for the individual to establish subjective distance vis-à-vis certain features of this identity.9

For example, the individual will now experience that portion of his identity that contains his anonymization as a 'worker' as being 'less real' than his identity as a private person or family man. Since each portion of identity relates to specific roles, it now becomes possible for the individual to perform some of these roles 'tongue in cheek'. The componentiality of identity, as the componentiality of social relations, makes possible an 'engineering' practice. This time what is involved is the 'engineering' of one's own self. Those aspects of identity that a defined as 'more real' must be protected against threats coming from the 'less real' components of identity.' p.37-38.

Helpful reference to ideas of multi-relational synchronization and institutional vectors and how our mind's became homeless.

Also encouraging, even though 'historic', discussion on life plans and life planning.

'A basic presupposition is that life careers are not firmly fixed but are at least relatively open. Put simply the individual is faced with a number of alternative careers, especially in his " younger years, and therefore must make decisions about these available options. It is possible for the individual to imagine, himself as having different biographies. This possibility has both positive and negative consequences for the emotional economy of the individual. Positively, it may give him a feeling of frees dom to shape at least certain parts of his life. Negatively, it increases the likelihood of frustration regarding specific careers One is, after all, less likely to resign oneself to a particular situation if one believes that other situations are, in principle, possible.' p.67.
'Implicit in this scale of probabilities is a general 'sociology', on whatever level of theoretical sophistication. The individual has a `map of society' within which he can locate and project himself in terms of both past biographical recollection and future projects. The individual's life is perceived as a trajectory across this 'map'. There is also a large body of factual knowledge, most of which is 'weighted' in terms of the individual's life plans. Thus if a particular career appears to the individual as a realistically plausible project, he will have more factual knowledge about it than about a career in which he can participate only in pragmatically irrelevant fantasy. In other words, a good deal of background knowledge is related to 'anticipatory socialization'. p.68.
'However vaguely this may be defined, there is the underlying concept of life plan, both for the individuals and for the a family unit. This life plan is the totalization of all the relevant timetables, their grand sum and their integrative meaning In modern society, such life planning has become a value in itself. Its absence is commonly an occasion for reproach. The family unit thus operates as a life-planning workshop.' p.69.

'There is a general expectation of justice. It is expected that everyone in the relevant category - as, for example, those en-titled by law to a passport - will receive equal treatment. It is, of course, understood that certain persons may be excluded from this relevant category - for example, resident aliens or convicted criminals. It is also understood that the bureaucratic procedures may codify certain preferences, for example, in favour of diplomats or other people travelling on urgent government business. Once the categorial system has been established, however, the presumption of equality holds within each category. In the most general way, bureaucracy is expected to conform to the Roman principle suum cuique. An implication of this is that there will be no favouritism or any other intrusion of personal bias in the bureaucrat's handling of each client's case. The bureaucrat is expected to handle every case sine ira et studio. In other words, there is the general expectation that bureaucracy will operate impersonally and with 'affective neutrality'.2 These considerations bring us to an important point: unlike anything discussed in the preceding chapter, the cognitive style of bureaucracy contains a moral quality, not just as a limiting factor (after all, there are also moral assumptions about technological production - for example, that no one will be killed in the course of it), but as an intrinsic part of its own structure of consciousness. The source for this moral element is probably to be sought in the primary social location of bureaucracy in the political sphere.' pp.52-53.

"Her life to date reads like something dreamt up during a Disney story conference: pretty American teen fulfils a life-long dream of a career in quantum physics at both Harvard and Oxford. And the fantasy cash register goes right on ringing because our heroine has not one career, but two. Between all-night stints in the lab she leads a double life as a professional ballet dancer.
At this point one the grown-ups in our imaginary brainstorm might lean in and suggest that maybe the two-career thing was a mite, er, far-fetched? But Dr Merritt Moore, quantum physicist and alumna of four major-league ballet companies (Zurich, Boston, Norwegian, and English National), is living proof that this crazy scenario can be lived in real time by anyone with the ability to devote themselves body and soul to the dreams that devour them." p.12.

c/o WHO: Opportunity for recent graduates for LDCs

Appealing to the librarian network to share this great WHO opportunity for recent graduates for LDCs

Dear Colleagues,

This may seem like an odd request but I am counting on you as the global health community who sometimes are the world’s best knowledge connectors. As many of you work with recent graduates who come from varied backgrounds, I would like to make you aware of the WHO’s young professionals (YPs) programme for young professionals (YPs) from Least Developed Countries (LDCs) https://www.un.org/development/desa/dpad/wp-content/uploads/sites/45/publication/ldc_list.pdf which aims to engage them in WHO’s work and build their capacity in key public health areas of particular relevance for their countries.

The vacancies of the first cohort are currently open for application until 6 December 2021. The vacancies can be found here:

https://careers.who.int/careersection/ex/jobsearch.ftl (please type “YPP” in the key word search to take you directly to the vacancies)

If you know someone who is 35 years old or younger and comes from a Least Developed Country, please ask them to check out WHO’s Young Professionals Programme! During your career, you may have come across some exceptional students from LDC locations who you feel worthy of an opportunity to make a big impact in the global health arena. If you know of anyone, please share this opportunity with them.

I have done this in the past and my colleagues in WHO HR saw star candidates results from the public health networking that got this message out to ideal candidates and they would like to see that success again. For some of you I know this will be a golden opportunity.

If you have any concerns about this posting, please let me know. For potential candidates, please direct them to visit the WHO website above.

Many Many Thanks to you all.

Always conscious of how Hodges' model, like many other 'models' is an idealisation. As such we routinely refer to 'social action', 'political process' and 'social process'.

I have, I suppose, allocated the four P's to the respective domains on a primary context - primary domain basis.^ So 'process' as per time, events, sequence, algorithm, logic, cause-effect.

"Dual Legacies of a Health Career and a Baton to Match" BMJ Evidence Based Nursing Blog

Approached through twitter I was invited to write a blog post on a mental health nursing related topic of my choice. The final text manages to reflect the time of year, the time of my career, student's careers and the health career.

There is of course a great deal of other very informative content.
