'Shattered' by Hanif Kureishi

Cathleen Schine, Ungovernable, Capricious Life, The New York Review of Books, April 10, 2025, Volume LXXII, Number 6, pp.15-16.

Shattered by Hanif Kureishi. Ecco, 328 pp., $28.00

'Shattered' c/o Penguin

'Hanif Kureishi, the English playwright, screenwriter, and novelist, posted these words on his Substack in January 2023:

On Boxing Day, in Rome, after taking a comfortable walk to the Piazza del Popolo, followed by a stroll through the Villa Borghese, and then back to the apartment, I had a fall.

I had been following Kureishi's Substack for a while out of admiration, curiosity, and loyalty-though Id met him only once, briefly. His dispatches were warm and entertaining, often about the writer's life. But this one was shocking:

I cannot move my arms and legs. I cannot scratch my nose, make a phone call or feed myself. As you can imagine, this is both humiliating, degrading and a burden for others.

According to his hospital report, his fall resulted in neck hyperextension and immediate tetraplegia. An MRI scan showed a severe stenosis of the vertebral canal with signs of spinal cord injury from c3 to C5. It was unclear, he said, whether he would be able to walk again "or whether I'll ever be able to hold a pen." He was speaking these words through Isabella, who was slowly typing them into her iPad.' p.15.

'A hospital is the place from which modesty and dignity disappear, desert you, seemingly forever, as soon as you cross the threshold. Kureishi, a reliably sexy, unrestricted writer, presents the physical deterioration and indignities he undergoes with his usual wit. The sense of vulnerability is crushing, but it is also one of the characteristics Kureishi reveals about himself that makes him so likable here, and the writing so intimate. The tone is remarkable: even the self-pity has no self-pity. One way he achieves this is by sprinkling punch lines throughout:

I woke up and started to cry. When you cry you must wipe away your tears, which is something I'm unable to do. So my eyes filled with bitter salty water and I got into a panic and thought I might lose my eyesight along with everything else. Finally, a kind nurse came into my room and downed me with a good dose of Lorazepam, then she touched me on my cheek and said, "It's not so bad, at least you're not in a coma."' pp.15-16.

Paper: What factors influence evidence use in policy? An updated systematic map

Abstract  [ REVIEW | Article in Press ]

In 2014, a large systematic map found that relational factors were amongst the most frequently reported facilitators of evidence use in policy. Since then, there has been a growth in evidence-use initiatives and research, many of which have attempted to address relational factors. In this timely updated map, we describe the current state of research about which factors influence evidence use in policy. A search strategy combining ‘evidence’, ‘barriers and facilitators’ and ‘policy/policymakers’ was used to identify studies across 15 electronic databases between 2012 and 2022. Studies were screened for those that include empirical data about evidence use in policy. Data extraction enabled descriptive analyses characterising studies in terms of focus, topic area, setting, and types of factors influencing use. Two thousand, one hundred and ninety-nine studies were included, a 20-fold increase over 10 years. The most commonly reported factors influencing use were the relevance of evidence, and then organisational (resources, skills, personnel and managerial support). A large number of studies reported on evidence-use interventions (n = 877) of which 379 were evaluations. Findings may indicate a shift towards relational and systemic thinking about evidence use. There appears to be a growing recognition of the resourcing, work, time, and workforce required to embed evidence use within and between organisations. With this huge growth in the literature, particularly in new evidence use interventions being tested across policy sectors, continued review and mobilisation is necessary to effectively inform the design and implementation of evidence use interventions and research.

Not a citation for Hodges' model, but I am grateful to Prof. Oliver and co-authors for being able to assist with data extraction using EPPI-Reviewer Screening Wizard. The preceding patience and guidance to complete the coding tasks was also invaluable in being transferable. I learned a great deal and it has been perversely reassuring (disheartening!) to see that the path to final publication is also 'circuitous' for academics.^  An additional lesson is building on existing work and publication (Oliver, 2014) to revisit a method and research question. (Updates to follow).

Previously: 'map' : 'systematic' : 'policy'

^A conclusion, much shared I believe: academic publishing is systematically dysfunctional.

Philosophical Counselling - Reading group: Taylor 2002 'Central Value of Philosophical Counseling'

In addition to Kantian Desert (this p.m.) it is proving helpful to join a reading group on Philosophical Counselling. Being able to combine clinical and therapy experience* with philosophy is a welcome and rather overdue development. The reading for tomorrow (online for me I hope) is:

Taylor J.S. (2002) The Central Value of Philosophical Counseling. International Journal of Philosophical Practice. 1 (2):1-9.

ABSTRACT: The title of this paper is deliberately ambiguous. It could refer either to the central value that philosophical counseling has for philosophy in general, or else it could refer to something (such as personal autonomy, or personal well-being) that philosophical counselors believe to be of value, and that they are able to help their clients pursue. In fact, this paper will be addressing both of these topics in order to demonstrate
the links that hold between them, and, in so doing, will attempt to further elucidate the nature of philosophical counseling itself.

Taylor (and related papers) also joins the dot (object?) that is Hodges' model, for example (with my emphasis):

'In this vein philosophical counseling is viewed as a Socratic drawing out of the knowledge that the client already possess, and that may be revealed through the careful questioning of a well-trained practitioner of the elenchic method.' pp.1-2.

I've long used guided discovery, by which the students (usually)/audience co-produce the model through a discussion.

'However, the difference between Socrates as a seeker after truth and the philosophical counselor as a professional who helps his or her client map out his or her own world-view in order to address particular problems that he or she is concerned with should not be overstated.' p.2.

'Furthermore—and perhaps most galling of all to philosophical counselors—academic philosophers often seem to view philosophical counseling as not being “real” philosophy at all, but more perhaps a species of social work, or an unsophisticated variant ofpsychotherapy.' p.4.

'Again, this information may be used by the counselor to help the client reflect more acutely upon her own views concerning the morality of abortion.' p.6.

'Given these two senses of “autonomy” it appears that the philosophical counselor is most concerned with enhancing autonomy in the second, Millian, sense of this concept. This is because philosophical counselors often try to aid persons in making decisions (or discoveries) about what it is that they really want to do, what their personal value-system or world-view really consists of.' p.11.
(A contribution to person-centredness? Critical thinking is also noted by Taylor.)

Share your voice: IRMS New Professionals Award

- entries close 31 January 2026

Dear students, apprentices and new professionals,

What are you up to between now and 31 January 2026? How about sharing your thoughts on anything to do with Information, Data or Records?

If you’ve not come across it, the Alison North New Professionals Award was set up by Alison back in 2010 to support emerging talent in our profession. 2025 marked its 15th year and the award continues to recognise and champion new voices.

Over the last fifteen years, entrants have offered everything from light‑hearted reflections to deeper insights into how we do what we do. Every applicant adds value, winner or not, helping the profession see familiar challenges in new ways.

What’s new for 2026:

We’re making submissions more flexible. Previously, you had to write an 1,000 word article.

This year you can still submit a written piece or you can record one instead (audio or video). As long as your entry covers 1,000 words’ worth of content it’ll be accepted and shared with the panel for consideration.

Why bother?

We know you’re busy studying or finding your feet in new roles. But for a little time and creativity, successful entrants will receive:

  • A funded place at the IRMS Conference 2026—a brilliant opportunity for content, networking and professional contacts. (IRMS 2026 is scheduled for 17–19 May at the Celtic Manor Resort, Wales.)
  • Publication of your winning entry to our 1,000+ members, and sharing across our partner networks and the wider profession.
  • Plus additional support to help you settle into—and thrive in—our fabulous profession.
So, ignore the inner imposter that says you’ve nothing to say. Set aside some time over Christmas and the New Year, and share your ideas.

For inspiration, our Patron, Scott Sammons, even interviewed recent winners on his podcast to explore why they applied—and why you should too.

(See https://theiglighthouse.podbean.com/e/special-episode-irms-new-professionals-award-carys-hardy/?token=dd9fdcd3d516c13f4874d8baca1c7b64 )

How to apply:

Deadline: 31 January 2026
Format: Written (1,000 words) or recorded (audio/video) equivalent
Details & submissions: https://irms.org.uk/professional-development/awards/new-professionals-award/

What’s stopping you?

Not a new professional yourself? Please share the application page with people who’d be a great fit and encourage them to apply—every little helps.

Trauma - MUSIC PRESCRIPTION - James Rhodes

'By now we're all familiar with the idea of music as therapy. For James Rhodes, however, music can be equivalent - at times even preferable - to  medication in the management of mental health. "Obviously, I am in favour of medication when it is necessary to save lives," says the 49-year-old British-Spanish classical pianist, when I meet him at Peregrine's, an upmarket piano shop in Clerkenwell, central London. "I was on medication and it genuinely saved my life. What I'm not in favour of is medication as a kind of easy option instead of doing other things that will also have the same effect. And there's no question that music [can]."
Manía, Rhodes's new album, is driven by that philosophy. Traversing swaths of classical territory, from JS Bach to 20th-century Argentine composer Alberto Ginastera, it is an attempt to profile the therapeutic properties of music from all angles, "I've always loved this idea of prescribing pieces," says Rhodes. As someone who has struggled with his own mental health, he says he has selected works that "accompany me and my insomnia, my anxiety, my desperation and my fears in the middle of the night"'. p.13.

'Amid a surge in demand for psychological support that has been exacerbated by the Covid-19 pandemic, Blue is one of many people without formal psychiatric qualifications — including volunteers and health workers — who are stepping in to deliver interventions.'

'Sangath became one of the first projects to run a randomised control trial (RCT) of task-sharing in mental health in 2007. “It’s not a compromise,” said Nadkarni, “it’s an idea backed by solid research evidence.”' [FT]

'Known as task-shifting or task-sharing, the trend has grown across healthcare as services grapple with a stark mismatch between resources and demand. The World Health Organization estimates that there is a global average of 13 mental health workers per 100,000 people, but in low-income countries this can fall to fewer than two.

But a number of studies suggest that simple interventions, such as benches where people can sit with a sympathetic listener, can be at least as powerful as expensive clinical approaches, easing the burden on specialist workers.' [FT]

'About us - Sangath: Our vision is a world where every person has access to quality and affordable mental health care.

Our signature principles are to leverage community and health system resources to deliver psychosocial interventions for the prevention and care of mental health problems. We address a wide range of mental health concerns across the life course, from cognitive development in childhood to self-harm and depression in youth, to drinking problems, psychoses and dementia in adults.  ...

Our name: ‘Sangath’ (सांगात) means togetherness and consensus. It signals our mission – to take an integrated and participatory approach to addressing mental health challenges.'

'Health policymakers are increasingly seeking to replicate projects pioneered in the developing world, a process that has been dubbed “frugal innovation”.'

'“Every $1 invested in a service like this gives you about $9 in return — both for the individual and the society combined,” he added.'

'Initiatives such as the Friendship Bench and the Common Elements Treatment Approach devised by Johns Hopkins University, a mental health intervention that can be delivered by trained and supervised lay providers in low and middle-income countries, are collectively reaching millions of people and have proved their worth in multiple studies.

But funders still tended to focus on setting up clinical projects rather than bringing about systemic change in resource allocation or medical payment models that would drive widespread adoption.' [FT]

Variable Hours Tutor: Take II

In September 2019 I was successful in securing a part-time post as a university tutor. This was great news.

While I've always worked in practice, I've enjoyed having students as a (sign-off) mentor and helping colleagues to fill holidays and other gaps; and completing a certificate in education (PGCE). In the past with the help of colleagues, I've organised teaching sessions for students on placement and small conferences. Now, I miss 'mentor updates' and meetings with the university outreach teams - practice education facilitators.

Of course at the end of 2019 a pandemic was brewing. The upshot - sadly for me - the tutoring was a non-event. No teaching (learning) opportunities followed, and the contract was not renewed in August 2020.

Through nurse revalidation in March 2023, I was helped in Warrington with another academic contact. I attended an open event, but again no openings. In (N) spring last year, I picked up the 2019 thread once again.

Success again! Another chance. Six months having passed, I do wonder though  - if any hours will follow?

Fingers still crossed, having chased and looking elsewhere too.

Be proactive: Evidence supporting proactive care for older people with frailty

'These BGS publications outline evidence and delivery recommendations for proactive care and support for older adults with moderate to severe frailty.

We set out how to deliver proactive care against core components and key enablers, with recommendations for success of a proactive care service. Building on NHS England's proactive care guidance, Proactive care: providing care and support for people living at home with moderate or severe frailty, we also outline key recommendations for the successful implementation of proactive care services across the UK.

We hope that users will be able to use all three documents when designing proactive care services, with the NHS England guidance document acting as key framework and starting point, BGS’s Be proactive: Evidence supporting proactive care for older people with frailty as a business case, and Be proactive: Delivering proactive care for older people with frailty  as a roadmap for implementing the NHS England framework and delivering services. ...'

"Hi! I'm new to Hodges' model - and this blog - what's it about?"

INDIVIDUAL
|
   INTERPERSONAL : SCIENCES
humanistic ------------------------------- mechanistic
 SOCIOLOGY : POLITICAL

The model was created in the early - mid 1980's with four purposes in mind - to facilitate:

1. reflection & develop reflective practitioners;
2. person-centred, integrated (holistic) care;
3. help bridge the theory-practice gap;
4. curriculum design, planning and development.

This was prompted by the change in the UK to university-based nurse education, which was to commence in (Project) 2000.

Learning and applying Hodges' model in 1987-88, and creating a website 1998-2015, I am clearly invested in Hodges' model and biased, questions and debate are welcome.

Created by Brian E.

Hodges' model is a generic framework -
a conceptual framework.

This model is situated, as 'generic' suggests
which means of course it is universal.

If you've a problem, scenario forthcoming project, or one that is nearing completion Hodges' model can assist in reflection and critical thinking to plan, scope, or evaluate as per your purpose.

This blog has posts about papers, events that interest me (PJ), plus many posts using this two-by-two table approach that suggests how the model can be applied in a variety of contexts.

This blog also has a bibliography
and template in the sidebar.

Hodges^

As noted previously (and last month!), it is marvellous to hear news of a paper citing Hodges' model. If you are planning a piece of writing, research and believe that Hodges' model may be of help please get in touch.

If you don't and can see merit in Hodges' model asit is presented, that is fine also. In fact, although I mention collaboration unique - untainted perspectives are probably much needed. Learning of a citation after publication is great news and encouragement.

This could and should be an amazing community:
Welcome to the QUAD

As a nurse who trained in learning disability, Brian Hodges was acutely aware of the political in health care, the role of the institutions and the aspiration of community-based and person-centred care.

Yes, you can use the model. It is open source.
All we* ask is that you cite and credit the source.

With the 'politics of health' much in mind there are many papers to write on the model that includes: reflection; person-centredness; integration of care; self care TO planetary health FROM ?; Inter- multi- transdisciplinary in Hodges' model; A model of healthcare viewed mathematically: Surely you're joking Mr Hodges?

This blog started in 2006, the template is dated, comments are disabled. A new site - repository is needed.

Young Carers Choir - 'Love is Enough'

'Young carers face many challenges throughout the year and Christmas can be an especially tough time.

When their friends are having fun, young carers’ responsibilities don’t stop. That’s why we’re delighted Sir Ed Davey and the Bath Philharmonia’s Young Carers Choir have come together to record for a wonderful Christmas song – Love is Enough.

The song, in support of young carers through Carers Trust and the Bath Philharmonia, has been written by young carers and shines a light on their lives.'

Comment: Menopause - a global health and wellbeing issue that needs urgent attention

On 'X', I noticed a comment piece from the Lancet:

Menopause: a global health and wellbeing issue that needs urgent attention
Gayathri Delanerolle, Peter Phiri, Sohier Elneil, et al. The Lancet Global Health, Volume 0, Issue 0.
Publication History: Published December 18, 2024
DOI: 10.1016/S2214-109X(24)00528-X

As usual I searched 'Welcome to the QUAD' [W2tQ] for previous posts tagged 'menopause'. The result '0' was a surprise, as there are several posts re.'menstruation'

https://hodges-model.blogspot.com/search?q=menstruation

And many tagged 'women':

https://hodges-model.blogspot.com/search?q=women

And this despite, a partner and family members experiencing the effects and impact of menopause. I was also asked in the summer to sort a list of signs & symptoms for menopause that can be associated with mental health nursing.

Interesting to see reference to the Roy Adaptation Model in the paper:
Klotzbaugh R, Fawcett J. Gender minority persons’ perceptions of peer-led support groups: a Roy Adaptation Model interpretation. ANS Adv Nurs Sci 2023; 46: 59–74.
And to address the global health extent and context of menopause:
'The stigma is such that open dialogue is inhibited in the majority of LMICs and the first-generation migrants from these LMICs to high-income countries (HICs); the paucity of broader understanding and acceptance in society exacerbates the mental and emotional strain of menopause and can lead to social isolation, particularly in these stigmatised contexts.' [my emphasis]
If needed there may be a CPD exercise for student nurses, or those qualified with revalidation pending? Hodges' model (the template) can help in achieving a broader understanding of menopause, and women's health to include menstruation and social attitudes, beliefs and attitudes. The paper does not refer to shame, and self-stigma, which may prevent a person from seeking help? So, there is a reflective exercise in reading the text and mapping the recommendations in the paper:

Panel - Recommendations to improve health outcomes for individuals with menopause
  • Lifestyle modifications
  • Mental health support
  • Holistic approaches
  • Culturally competent care
  • Personalisation of hormone replacement therapy care
  • Peer support groups or social prescribing
  • Conduct comprehensive research
  • Improve accessibility
  • Understanding various risk factors, such as lifestyles, environment, and climate
If anyone has events, journal special issues for 2025 ... I can help publicise, please let me know.

On behalf of the Vice-rector for Research and Director of the Institute for Interdisciplinary Research (IIIUC), Professor João Ramalho-Santos, we would like to inform you that applications are now open for the 6th edition of "Seed Projects for Interdisciplinary Research", with the support of Santander Foundation, promoted by the UC Strategic Areas Unit (NAE), in collaboration with the Institute for Interdisciplinary Research (IIIUC).

Through this call, the UC intends to provide support for the initial development of original and interdisciplinary research projects with seed funding that will allow UC researchers to make their scientific proposals more robust, ensuring competitive funding in the future and the development of new research lines that cross areas of knowledge at the University of Coimbra.

There will be funding for up to five interdisciplinary research projects, one in each of the UC Strategic Areas: Health; Climate, Energy and Mobility; Natural Resources, Agri-food and Environment; Digital, Industry and Space; Heritage, Culture and Inclusive Society.

Researchers of the R&D Units (or other structures of the University of Coimbra), who carry out research at the UC and who obtained their doctorate after 1 October 2014 are eligible to apply for this funding. The project team must include three to six researchers from the UC, from at least two R&D Units in
different scientific fields.

The applications can be submitted until 6 December 2024 at 5pm on the call website:

The call guidelines, proposal template and the list of juries appointed for this edition are available on this page.
