“Instagram helped kill my daughter. Too little has changed, promises to deliver safety have failed and algorithms still show streams of toxic content to children. We need to protect our children from the profit-driven business models that powers tech companies and puts the safety of children at the bottom of their priorities.”

Choose to Treat appeal, 7–14 July 2026

Donate here between 7–14 July and your donation will be doubled!

'Water is scarce in Mauritania and people have little choice but to bathe, wash clothes, care for their animals, and cool off in the same pools. But the water that brings relief also carries schistosomiasis, a parasitic disease that silently damages the kidneys, stunts growth, impairs learning, and steals futures.

Through our collaboration with the Ministry of Health, Amadou (pictured*) received treatment, and he recovered. But many children in Mauritania still require it. Without continued access to treatment, Amadou and thousands like him remain at risk every time they enter the water.

When people cannot avoid exposure, access to safe, effective treatment is their best protection. A single donation can help protect children like Amadou and support their health at a critical stage in their childhood.' ... 'Choose to Treat appeal'

RCN Congress: Ebola, privacy and 'care bundles'

The first discussion at RCN Congress #RCN26 on Nursing as a STEM profession, or STEAM, with the arts added, became a resolution:

https://www.rcn.org.uk/congress/congress-events/nursing-as-a-stem-profession-2026

This was marvellous as it touched so many aspects that Hodges' model readily encompasses.

There is scope for the addition of emergency items:

https://www.rcn.org.uk/congress/Submit-an-emergency-agenda-item

Yesterday I wondered about the latest ebola outbreak, national and international aid^. Plus, privacy, having noticed a stream on twitter regards the NHS and a contract with Palantir. Yes, 'X' as a source is a problem, but concerns about access to confidential clinical information is found in the quality press. What does the Information Commissioner's Office say? Well, it seems there is a history:

https://icosearch.ico.org.uk/s/search.html?collection=ico%7Esp-search&query=PALANTIR&profile=_default

While searching for 'care bundles' I came across this:

'The scope and force of medical privacy is further supported by a secondary layer of protections based in sectoral and other forms of regulation. Anti-discrimination laws, disability rights, and employment law, for example, all recognize and operate to preserve medical privacy by restrictions of use of medical information outside of the medical setting.

These intersecting normative strands of medical privacy, derived from different sources, together form a set of norms designed to protect a bundle of interests that is essential to the maintenance of an effective healthcare system that encourages and protects appropriate care-seeking and treatment. Whether and how technological changes in the collection, storage, and processing of data affect the construct of medical privacy is a pressing question. Just as a bell cannot be unrung, erosion of the sphere of medical privacy is unlikely to be restored.' p.330.

I realised over the past twenty years or so I've paid less attention to the fact that sections of a population, those often most in need of healthcare will not engage, be care-seeking - if they have no faith and trust in how their information is managed and protected. Minority groups, the digitally excluded (whether by choice, or literacy, economics) the influence of media are all vulnerable. Protection of data and the integrity of health services and systems is surely a matter of professional import both clinically and informaticians? As a registrant, I've always put it to student nurses that they should be aware of what happens to the data they as a registered nurse, enter in the electronic health record.

^What, only one sentence? Yes, but I'm still worried; geography - distance now matters little. While, on the other-hand: care-seeking...?

But, nobody will bother me now!

I don't think this is right.

Either way, I'm asking for trouble?

Will anybody else listen?

Should I risk trying to buy a knife?
How? Shop, or online?

Do I look 18?
I've seen the signs by the tills.

Can I pass as an 18 year old?
Should I pick it up and take it?

Or, do I leave it exactly where it is hidden?

Do I really need a knife,

'Apps such as TikTok and Instagram are filled with videos of young girls and teenagers advising their peers on elaborate skincare routines, especially through a popular line of content called "Get Ready with Me".

Parents have sounded the alarm over this trend, while recent medical literature has recognised the phenomenon of "cosmeticorexia", also called "dermorexia", defined as a culturally reinforced preoccupation or obsession with achieving "flawless" skin that can lead to "excessive, age-inappropriate or compulsive use of cosmetic products".'

'Italy is investigating a beauty retailer and make-up brand owned by French luxury group LVMH over claims their marketing has fuelled "cosmeticorexia", an unhealthy obsession with skincare among young girls.
The Italian Competition and Market Authority (AGCM) said yesterday it was probing Sephora and Benefit Cosmetics for suspected "unfair commercial practices" with the promotion of skincare products such as face masks, serums and anti-ageing creams to girls as young as 10 years old.
Those marketing campaigns were carried out mainly via social media, the  regulator said, leading young girls to "compulsively" buy such products.'

New student? What's that? You're worried about your finals!

For a new student nurse, mention of your finals, as in, exams - seems a world-away. After a paroxysmal attack of fear (and motivation!), time resolves. "It's fine. I can relax. We're good!"

Especially so in the pre-Project 2000 days (Swaby, 2021), when nurse education was conducted in more local schools of nursing. You could be forgiven for feeling reassured that there's plenty of time; if your first step was to work as a nursing assistant (or cadet) prior to starting as a student.

Then all of a sudden, you can point to many wornout shoes. You've been busy; helped run a ward, a caseload, a team, and raised a family. And, while the NHS, nursing, society, and much else besides have continued their relentless pace of change, change and more change.

You blink,* look again and you're starting what will be your final 3-year period of nurse registration. What this will bring: who knows?

BBC Radio 'File on Four' - DASH to Instrumentalise

Of course, I'd like to see Hodges' model applied to many purposes.

As probably posted at some point, the thought of Hodges' model being instrumentalised goes back a long way. To the time of microcomputers in fact and 'CAPA' which stood for 'Computer-Aided Patient Assessment':

Jones, P. (1986) Computing in Nursing NEWS. Computerised Patient Assessment. Nursing Times. 85: 5. Sep 3-9;82(36):63-5. PMID: 3532039

[Describes 'CAPA', a BBC BASIC microcomputer program for student nurses.]

Even before discovering Hodges' model in the late 1980s, I was sensitive about being seen to mechanise nursing and care delivery. Nursing was still preoccupied with the nursing process and individualised care. The last thing I would want is to be seen as supporting the processing of patients. The person, their social context and identity becoming lost. It was a long-term and ongoing co-author who drew my attention to Hodges' model as an instrument. Which I've considered for each and across the care domains. CAPA was simplistic, even beyond the programming language employed. A dependency score from 1-5 was allocated (it's a long time ago!) to each of the activities of daily living for a patient:

Roper, N., Logan, W., & Tierney, A. J. (2000). The Roper-Logan-Tierney model of nursing: Based on activities of living. Churchill Livingstone.

In healthcare we are trained and become accustomed to the use of many assessment tools, checklists; this includes mental health nursing and specific therapies. Emphasis, is rightly placed upon the safety, validity and hence testing of tools. Some are 'broad brushes'. Others are specific - outcomes, anger, a carer's knowledge, conviction of belief, mood, impulsivity and many more. Drift away from the original goal and purpose and problems can (predictably) ensue. Training will and must be updated, but do all assessment tools have a 'Use By' date? Hopefully there is ongoing development, version control and sustained trials and testing of the instruments themselves.

'File on Four' BBC Radio 4 this evening concerns DASH, which stands for "Domestic Abuse, Stalking and ‘Honour’- based abuse".

NOT 'Nanny State': Protection & assurance of the quality of nutrition

The Government needs a plan to fix our broken food system and turn the tide on the public health emergency

24 October 2024

The House of Lords Food, Diet and Obesity Committee demands that the Government should develop a comprehensive, integrated long-term new strategy to fix our food system, underpinned by a new legislative framework. This is the key conclusion of the Committee’s report, ‘Recipe for health: a plan to fix our broken food system’.

Chair's comments

Baroness Walmsley, Chair of the Food, Diet and Obesity Committee, said:

“Food should be a pleasure and contribute to our health and wellbeing, but it is making too many people ill. Something must be going wrong if almost two in five children are leaving primary school with overweight or obesity and so many people are finding it hard to feed healthy food to their families. That is why we took a root and branch look at the food system and analysed what had gone wrong over the past few decades.

“Over the last 30 years successive governments have failed to reduce obesity rates, despite hundreds of policy initiatives. This failure is largely due to policies that focused on personal choice and responsibility out of misguided fears of the ‘nanny state’. Both the Government and the food industry must take responsibility for what has gone wrong and take urgent steps to put it right.

“We hope, given the recent comments from the Prime Minister, Lord Darzi and the Secretary of State for Health, that there is now an appetite to shift towards prevention of ill health. We urge the Government to look favourably on our plan to fix our broken food system and accept that not only is it cost-effective, but that it would lead to a lot less human misery.”

[My emphasis]

About time! Real action please.

DO NOT DILUTE TO TASTE!

Comparison and contrasting the Meikirch Model and Hodges' model

Learning of another conceptual framework, which helpfully is also described as a model, there is a website, publications and illustrations (well executed PNGs). Enter, the Meikirch Model:

Bircher J. und Hahn E. G., Will the Meikirch Model, a New Framework for Health, Induce a Paradigm Shift in Healthcare? Cureus 9(3): e1081. DOI 10.7759/cureus.1081

The title of the paper is impressive, and on reflection worthy because we really do need a paradigm shift in healthcare.

How to achieve universal health coverage and do so sustainably? In turn, how to realise the Sustainable Development Goals, while having regards the dependencies, determinants of health? For Hodges' model this is factored in by Brian (Hodges') recognising the political in situations including health. (So, I would argue that the required paradigm shift extends much further, but more on that to follow.)

Below I will review some of the main arguments, facets, and principles of the Meikirch Model, drawing from their website: https://meikirch-modell.ch

Meikirch Model: Overview

The Meikirch begins with 'demands of life':

"Each living creature has to fulfill the demands life puts on him or her. In man these demands include biological, psychosocial and environmental shares."

From the outset the INDIVIDUAL being - as a living creature (hence animals, flora too?) is highlighted - as per the individual - self, in Hodges' model. Maslow's hierarchy is presented too in stressing the need for shelter to counter the demands of the environment.

I like the way the individual draws upon 'biological given' and 'personally acquired' "potentials". In Hodges' model the individual's interpersonal development, their intellectual abilities, motivation, attitude, personality and more all contribute to a person's self-actualisation.

Demands of life provides the basis for the Individual in the Meikirch model, as per the figure. In the notes the responsibility of the individual in the extent to which they achieve their potential appears to be emphasized. Responsibility extends to:

  • how he or she cultivates the relationship to other human beings.
  • how she or he participates with the life of the society.
  • how he or she deals with the natural environment.
There is acknowledgement of how the biologically given potential can be reduced, and so:
"may compensate their defects to a large part by further developing their personally acquired potential. "
The Meikirch model is "A new concept of health based on science".

For the Individual, an example is given of type 1 diabetes were the person is able to become an proficient in management of their condition, possibly even an expert - and most likely the expert barring complications (infection ...).

I've blogged (I think!)* and Twi/Xed about the old joke (of course, it is not) of the N.H.S. being a misnomer. It really stands for the "National Ill-Health Service". Since drafting this in February, I'm sure I've heard this again on the radio, amid the ongoing UK election process. The website notes how generally:
"The health care system is 'sick'

This sickness results from a concept of health that is blind itself."

It is also not just just the need for change, as stressed by Meikirch; but how to make the transition to health education, promotion and prevention? Meikirch is a marvellous addition to the argument.

Moving further another common component with Hodges' model is the SOCIETY and social determinants in the Meikirch model, Sociology in Hodges' model. As Profs Bircher and Hahn note, relationships are key:
"The social context of a person encompasses the family, friends, acquaintances, schools, employers, including political institutions. Good lively personal relationships in family and social settings promote health. In contrast, burdensome relationships may compromise health. Whoever damages human relationships, exacerbates the development of the personally acquired potential of affected persons including his own."
The role of the law in the social framework is acknowledged, and how anxiety, greed and abuse can obviously impact on an individual's health. This extends from within the family, community, to the workplace; relationships between employees and managers for example.

Helpfully, society is also responsible for the health of (its) individuals. But this also relies upon mutuality with individuals:
"each individual must also contribute his or her part for a proper functioning of the society."
A reference that links the collaborative nature of society with public health:

Johannes Bircher, Shyama Kuruvilla, Defining health by addressing individual, social, and environmental determinants: New opportunities for health care and public health. Journal of Public Health Policy (2014) 35,363-368. (with link).

The ENVIRONMENT makes up the outer-most ring in the diagram. The Meikirch author's connection with Switzerland is evident in the protective examples of avalanche barriers and a natural deficiency of iodine and fluorine.
"iodization of cooking and table salt and fluoridation of tooth pastes to prevent thyroid diseases and dental caries. Construction of avalanche barriers and of houses resistant to earthquakes also protects health."
Sadly we have seen backward steps in dental services provision in the UK; and recent earthquakes have demonstrated the tragic consequences of lapse building regulation when buildings 'pancake' in a progressive collapse.

I contacted the authors https://meikirch-modell.ch/en/authors/ but no response has been received.

I will reflect further - adding a table with concepts related to Hodges' model and discuss the table at: https://meikirch-modell.ch/en/model/

Historic Climate Ruling: States Must Step up Climate Action to Protect Human Rights

From: Center for International Environmental Law (CIEL) PRESS ROOM ...

STRASBOURG, FRANCE – Today, the European Court of Human Rights issued a groundbreaking ruling in the Verein Klimaseniorinnen Schweiz and Others v. Switzerland climate case. The Court found Switzerland in violation of the European Convention on Human Rights for failing to implement sufficient measures to combat climate change.

Center for International Environmental Law (CIEL) Senior Attorney Joie Chowdhury issued the following statement in Strasbourg, France:

“We expect this ruling to influence climate action and climate litigation across Europe and far beyond. Today’s historic judgment in Verein Klimaseniorinnen Schweiz and Others v. Switzerland – the first ruling by an international human rights court on the inadequacy of States’ climate action – leaves no doubt: the climate crisis is a human rights crisis, and States have human rights obligations to act urgently and effectively and in line with the best available science to prevent further devastation and harm to people and the environment. The ruling reinforces the vital role of courts – both international and domestic – in holding governments to their legal obligations to protect human rights from environmental harm. It also affirms the power and courage of those who speak out and dare to demand a livable future for all.”
