News on two papers: Deprivation of Liberty Safeguards & Threshold Concepts

 - plus of course Hodges' model


While I could find out (here on W2tQ!?), I can't remember exactly when I started a writing project about threshold concepts, deprivation of liberty safeguards in the context of residential care and nursing homes. It was at least 2013, and may date back to 2011. Well, whenever it was: what a palaver! 

One paper, rejected, the advice was to split the work and create two. The two papers were then also rejected. My writing is always off-piste as it were, on-off too; and then ironically COVID-19 may have helped too? Afterwards, I started to pick it up again - the same title but parts 1 & 2. I never heard from one journal, so quickly moved on. Each time, the work improved thanks to two reviewers and their feedback. All the time my thought about Hodges' model has itself changed: I like to think progressed. With further revision, I finally had two standalone papers.



The important thing is the news received this week: Re. ...
'"A Conceptual Mapping Exercise of Deprivation of Liberty Safeguards in Residential & Community Care using Hodges’ Model and Threshold Concepts". I am pleased to be able to tell you that has been accepted for publication in Journal of Evaluation in Clinical Practice.'
A further paper (part 1 originally) same journal, to be referenced in the above, needs some minor revisions.

I am worried about another paper, with a similar history, that I can now turn my attention to (this itself speaks volumes!). I can see I need to alter the emphasis, balance of Hodges' model, diagrams, psychological therapies and case formulation. A co-author would help, but of course everyone's busy.

So if you're writing and struggling: do keep going!

Take II? Fit for Government . . ?

As a community nurse, driving is obviously pretty essential to your being able to work. I have come across non-driving community nurses (earlier this year) but they are still few(?). As a boy going to work with my father you could see how dependent he and workmates were upon a string of vans, cars and be able to drive both being on the 'right-side' of the law and his health. It was always an 'event' a new van, fitting out with racks, shelves, nooks, hooks and crannies for all sorts.*

Then as a community nurse older adults you come across individuals who still have their driving license, still have their car on the drive / front, and still have the car keys. Perhaps family have suggested to mum, dad, brother, sister, aunt: "Look, isn't it time to call it a day and give up driving?" Ouch! There's a crash - in itself.

Re-working the two-part draft paper on deprivation of liberty safeguarding, threshold concepts, Hodges' model within residential care, some thresholds are difficult, challenging to call. But some are safety critical, and perhaps need a more 'proactive' stance, without giving way to ageism. Is it the local community's job - the public's job to proffer a diagnosis: and what about persons on the world stage? Diagnosis by social media?

I quickly admired those people who, though reticent, gave up the literal fast-lane. Inevitably you wonder how you will react when the time comes. Will you have the humility, the goodness of grace to hand over the keys? To call it a day. With an ageing population, I've warned the children when driving to expect the unexpected. Cars going the wrong way. It was a struggle for my father: but safety as it so often does - did the trick - with the impetus of physical illness.

Sometimes, at work, after a family trying, and a fracas, when they've tried to hide / remove the car keys, suddenly a near-miss has upped-the-ratchet. Neighbours have seen the 'driver' reversing out into the road, with numerous car horns announcing the event. Then perhaps you are intercepted outside, a call comes to the day center (the transport is also going), the community team, or the GP's surgery (the neighbours have helped with medication).

I've had to speak to GPs for them to visit, meeting them on occasion. Do GPs - family physicians still have the same gravitas today?

This is a care transition: what can be a difficult one. A helpful point (diplomatic argument) that often gets through is the matter of car insurance and the status of the existing policy?

Does a political leader carry insurance?

It is difficult for all, a person's human rights, choices, freedom; their life story, former career perhaps, their health career - most definitely.

Reablement, Rehabilitation, Recovery: Everyone’s business c/o BGS


In March 2023, we published Joining the dots: A blueprint for preventing and managing frailty in older people. This set out seven touchpoints of care that should be available to older people, as and when they need them and 12 recommendations to systems. One of the recommendations is specifically around the provision of rehabilitation and one of the touchpoints is ‘Integrated urgent community response, reablement, rehabilitation and intermediate care.’  

In this new document, we take a deep dive into this Blueprint recommendation, looking at the evidence behind the provision of rehabilitation for older people and highlighting examples of best practice from across the UK and internationally. We argue that rehabilitation should be a component of virtually all care for older people, across all care settings, and that there is no such thing as ‘no rehabilitation potential.’ 

This document has been led by Professor Anne Hendry, Honorary Professor at the University of the West of Scotland and Senior Associate at the International Foundation for Integrated Care and a group of BGS members from across the UK and across disciplines have contributed to drafting the document. We hope that BGS members, commissioners and system leaders will find it useful in planning rehabilitation services for older people in their communities.

Historic Climate Ruling: States Must Step up Climate Action to Protect Human Rights

STRASBOURG, FRANCE – Today, the European Court of Human Rights issued a groundbreaking ruling in the Verein Klimaseniorinnen Schweiz and Others v. Switzerland climate case. The Court found Switzerland in violation of the European Convention on Human Rights for failing to implement sufficient measures to combat climate change. 

Center for International Environmental Law (CIEL) Senior Attorney Joie Chowdhury issued the following statement in Strasbourg, France:

We expect this ruling to influence climate action and climate litigation across Europe and far beyond. Today’s historic judgment in Verein Klimaseniorinnen Schweiz and Others v. Switzerland – the first ruling by an international human rights court on the inadequacy of States’ climate action – leaves no doubt: the climate crisis is a human rights crisis, and States have human rights obligations to act urgently and effectively and in line with the best available science to prevent further devastation and harm to people and the environment. The ruling reinforces the vital role of courts – both international and domestic – in holding governments to their legal obligations to protect human rights from environmental harm. It also affirms the power and courage of those who speak out and dare to demand a livable future for all.”

LIVING WITH PRACTICAL REALITIES, 1978

"Stephen Willats aimed to explore the realities of living in a British tower block. The work centres on Mrs Moran, an elderly woman who lived at Skeffington Court in Hayes, West London. Willats photographed and interviewed Mrs Moran over the course of six months. The text in the work is based on these interviews. In his composition, Willats highlights the physical, social and economic constraints that she faced. Each panel also features a question. These invite the viewer to participate directly in Mrs Moran’s lived experiences."

BGS Report "Smarter data, better care: Empowering care homes to use data to transform quality of care"

"The British Geriatrics Society (BGS) hosted an event in London in September 2023 on ideas and practice around a minimum dataset for care homes. This report, which is based on presentations at the event and the debate that followed, makes 12 recommendations for the effective implementation and delivery of a national minimum dataset that we believe policy-makers and regulators should consider."

*Hodges' model is ideal for initial scoping, planning, and holistic assurance throughout research, study processes and (case) formulation. The model provides ready-to-mind access to conceptual inter- multi- transdisciplinary bridges (even as we struggle to define the latter), e.g. socio-technical. 

Immediately preceding the above quotation, Karvonen and Saariluoma write:

"Arriving at this point via the cognitive mimetic route yields a deep and rich picture of human-technology co-agency in industrial processes." p.17.

A 'rich picture' (Checkland) is a feasible output of Hodges' model (extending Karvonen and Saariluoma's context). Clinically, this might comprise a 'comprehensive' assessment, or case formulation. It is bounded, guided by being situated, while the clinician's experience and patient's is built up across several encounters (appointments/home visits...). Unpopulated, initially (please see the template in the sidebar) the model serves as an aide-mémoire, an inner critical friend, helping to sustain values, and person-centredness. At a one-to-one(-group) level  integrated care, that is, parity of esteem across physical and mental health, a professional attitude and disposition to practise through unconditional positive regard remain key competencies. Each contact can act as its own 'reset', another blank sheet/slate: what can we learn together on this occasion? In some contexts this 'pause' can assist in risk management and safety - ensuring that the rich picture, and the knowledge gained thus far, does not mean things are taken for granted.

This morning I shared three slides with the PAMHOP research team and greatly appreciate the opportunity to present and join their meeting:

As I spoke of considering the environment and the carceral context across the domains of the model, I recalled another piece of paper.

In prison of course, while the physical environment is the intended constraint, the model also invites consideration of the psychological, social, political environments and added here - the spiritual.

Since 2019 and post-COVID, the change in how we regard and value the spaces we inhabit - voluntarily or otherwise.

"A nurse who was among the first in the UK to blow the whistle on safe staffing issues has died aged 91.

Graham Pink first raised concerns about poor staffing and the dangers to patients when he worked at Stepping Hill Hospital in Greater Manchester in 1990. His four-year campaign to highlight inadequate staffing levels made a lasting impression on the nursing profession. Mr Pink died on 6 March in Barnsley, South Yorkshire.

Nurses caring for him remembered his safe staffing campaign

Speaking to Nursing Standard about his final months, his niece and carer Sarah Pink said: ‘Barnsley Hospital gave him a lot of care and attention and treated him with a huge amount of respect. There were nurses caring for him who said they knew him from what he did and how he stood up for the NHS back then. One sister said it was an honour to meet him and take care of him.’"

Kirigami: Engineering in a World of Friction

Bioinspired kirigami metasurfaces as assistive shoe grips

Abstract

Falls and subsequent complications are major contributors to morbidity and mortality, especially in older adults. Here, by taking inspiration from claws and scales found in nature, we show that buckling kirigami structures applied to footwear outsoles generate higher friction forces in the forefoot and transversally to the direction of movement. We identified optimal kirigami designs capable of modulating friction for a range of surfaces, including ice, by evaluating the performance of the dynamic kirigami outsoles through numerical simulations and in vitro friction testing, as well as via human-gait force-plate measurements. We anticipate that lightweight kirigami metasurfaces applied to footwear outsoles could help mitigate the risk of slips and falls in a range of environments.
