Arriving at Hodges' model via guided discovery ...

Following Brian Hodges’ lead in his notes, I have engaged a great many learners with a guided discussion through the following questions:

Questions	Answers	Outcome/Significance
Who do we care for?	The person, individual, patient, client, carer.The group, family, community, national/global population. The 'duty of care'. The Planet.	It is the individual (guardian) who consents to care. The subject at the top of the vertical axis of Hodges’ model is first identified. It is the person who consents to care (or their guardian); then the group occupies the bottom of the vertical axis.
What do we do to help deliver care?	We attempt - complete tasks and interact in ways that are both mechanistic and humanistic. We use procedures, equipment, devices, technologies, physical interventions; and the self – empathy, rapport listening, being present. Team work with effective communication, coordination and collaboration are also essential.	To care is to act. The horizontal axis is added, with humanistic to the reader’s left; and mechanistic to the right. The axes are then complete, revealing the model’s ‘fixed’ structure. Interdisciplinary can be identified and further discussed.
What skills and knowledge do we need to care for the individual, family and populations?	Here we are concerned with the content of the given situation. This can encompass the whole curriculum and acknowledges experiential learning.	The epistemological and ontological scope and scale of health is revealed from self to planet. The importance of context and situation can be stressed. Including first-aid and its rules and risk assessment. Discussion visits and demonstrates the significance of each of the domains of Hodges’ model and the spiritual to participants. The relevance of Hodges' model to the 21st century, as a bio-psycho-socio-POLITICAL model is emphasised.

The Line(s)

Foresight, hindsight, sight itself and that illusory association with the line of time is a fascination for us all. No doubt, as a result of the hardwired analogue found in the lifeline and our ultimate fate. Hodges' model acknowledges all these lines; concrete and abstract. The work and health careers, an individual's education career (where accessible / permitted) now lifelong learning.

Abstract things seem tenuous, indistinct, maybe lost somewhere in theory. Concrete on the other hand, is physical, tangible. 

Hodges' model can cover paper with lines, two of them ideally. The model can then be applied in practice.

There is then an interplay between abstract and concrete; between which data, information and knowledge flows, uncertainty reduced. Scope and scale become clearer once assessment, planning, intervention and evaluation are concurrent.

This applies to all lines, if they are not destined to be covered in sand.

Global health is ... c/o King & Koski (2020)

Use of the word 'global' in health care is common: from global assessment; global as in a pandemic, as per Covid which for a time closed down most human activity across the world; the global health workforce; and global health crises, that must include the climate crisis and pollution. Universal health coverage, allied with universal healthcare access, refers to (national and aspirational ...) global access to quality health services. 

Above, 'global health' has itself already occurred several times. I've posted about global health on a great many occasions on behalf of other parties.

Defining global health in the context of public health, King and Koski (2020) write:

'We propose the following definition:
global health is public health somewhere else.'

More specifically, they add: 

'Global health as a field is not distinguished by its aspirations, methods of research and practice, intervention strategies or even geographical area per se, but rather by a particular relationship between its practitioners and its recipients: a person engages in global health when they practise public health somewhere—a community, a political entity, a geographical space—that they do not call home.'

To apply Hodges' model to global health, there must be another definition.

AI - World Models

World models are neural networks that understand the dynamics of the real world, including physics and spatial properties. In healthcare a 'world model' is slightly more expansive, hence the importance of experienced humans. We call these - nurses, doctors, physiotherapists, occupational therapists, social workers and many other professions, support workers and disciplines. They learn and train for many years and must continue to learn and unlearn throughout their careers. Their work and engagement is shaped and directed by human values, which are in turn informed by social change, evidence-based research, professional guidance, policy and law.

RCN Congress 2025 ii - Losing direct nursing skills

No longer an issue for me, but in the mix of careers: health, education, work - professional; employees often find that they are pulled away from what originally brought them to their chosen career. "I want to help people, care for them, make them better." Especially, if they are ambitious, seek better pay and prospects, for themselves and (a) their family. 

This was an issue and tension for discussion at the RCN Congress:

'Climbing the ladder often means leaving the “bedside”, this is something that can leave mixed emotions for the individual. As a result, we are losing years of clinical experience and expertise which could impact on the future training and on-the-job development for early career or inexperienced colleagues.

The secondary issue that arises with nurses leaving the bedside is skill decay, where a person loses the skills developed in the clinical area through lack of everyday practice. This is being increasingly acknowledged in health care literature (Maehle, 2017). However, the biggest barrier is the resulting lack of confidence in personal skills and ability, particularly where someone is now seen as a senior colleague by those at the bedside.' cont. ...

Discussion - Losing direct nursing skills

I didn't make notes but reflected on how your 'basic training' (for me ...) registered mental nurse (RMN) and state registered nurse (SRN) provides the foundation for professional (and lifelong?) learning? This is, after all, what continuing professional development and post-registration refer to. I did realise that in returning to mental health nursing after qualifying as a SRN, I would surrender much learning - instead of consolidating.

Lecture 101, sets in train the scope of your theory and practice. This is why, and where I see Hodges' model having a role at this point in our professional education, whatever the discipline, or profession. Congress included a resolution for debate: Physician associates. 

We really do need to get this right. Get it wrong, and the quality of care and safety of the public are threatened. If existing standards and professional relationships are perturbed, the effects can be long-lasting and (grossly) injurious. Especially at a time of increasing demographic pressures globally for health systems and services.

Peter Higgs - Boson and Field

"Higgs theorised that particles would acquire mass by interacting with a new type of field. That field had a very special excitation of its own, another particle called the Higgs boson. The Higgs field solved a huge question in theoretical particle physics, and the Higgs boson was a tantalising target that experimentalists could hunt for in order to tie theory to reality.

"Whilst the impracticality of unified approaches has been noted for some time (McFarlane, 1976), it is still considered that many theories, in their efforts to explain everything, succeed only in explaining nothing (Draper, 1990). Therefore, as Kenny (1992) points out, the use of theories and models in nursing has resulted in sweeping generalizations which 'are not always personally, culturally or contextually appropriate'.

In overcoming this difficulty many authors advocate that theories of lesser scope and abstraction are considered (McFarlane, 1976; Clarke, 1986; Draper, 1990; Moore, 1990; Ingram, 1991; Reid & Bond, 1991). Such theories have been termed 'mid-range' and address a more limited number of variables in particular situations (scope), whilst being empirically grounded and focusing on practical problems (abstraction) (Rogers & Shoemaker, 1971; Walker & Avant, 1983; Fawcett, 1984; Lowenberg, 1984). According to Clarke (1986), mid-range theories should appeal to practitioners as being more directly accessible conceptually and linguistically. Reed & Robinson (1991) contend that, given the diversity of nursing practice, the search for grand theory is inappropriate and nursing would be better served by developing mid-range theories that are 'more precisely stated, more easily treated and produce more specific indications for practice'." pp. 59-60.

While the structure of Hodges' model is global - 'grand' in scope, the model, as a template is blank and open to whatever conceptual content follows in the practice situation, or context. 'Precision' can be assured using Hodges' model as it can facilitate person-centred care, integrated care, reflection, critical thinking and conceptual development. 

Delivery of high-quality health care is not a given. 

Safe, effective and equitable care is variously dependent upon staffing, the skills and attitudes of individual nurses, staff numbers, skill-mix, team effectiveness and resource allocation. Hodges' model can also assist users to recognise and call-out unmet needs, deficits in the quality of care, and risks to the safety of patients, the public, staff and the profession.

At risk of sounding grand if not grandiose ...

Change will and must follow - in respect of models and theories, not just of and for nursing but health and care at scale.

Update on draft papers iii

There was news today about the draft paper on:

"First Episode of Psychoses, Early Intervention, Cardiovascular Nursing, Research Methods and Hodges' model."

It has been accepted and I can submit formally. 👍

This project - yes, a further distraction (that has borne fruit) started in March resulting in 4,800 words, an illustration of the model and 4 figures (2x2 tables). That I think, with the editors, guide the reader in applying Hodges' model, or at least the scope of its application, which is situated and person-centred.

I approached a journal regards the draft "Society, Technology and COVID19 in Hodges' model". Having reduced the original 12000 words to 7700, the paper is of interest, but is (as expected) too long. There is the possibility of reducing the word count to 5000, or splitting the essay.

This invites revisiting the original and teasing out the informational themes: fake news, mis- dis- malinformation and agnotology (in part 2?). Then I would (to a degree), finally be returning to the concept of 'information':

Memo: Understanding the difference between 'self-care' and 'SELF-CARE'

As we learn in infancy, run into physical structures and it hurts. The presence of structure in this case clearly has impact - not just literally - but globally.

While for me, the model is a constant, I'm constantly surprised by the 'work' that its structure, the two axes - can do from the initial - blank - template (empty set):

Throughout history, many utopian visions and theories have claimed to take a “global” or “universalist” approach, i.e. to speak for the whole of humankind. As such, utopian thought, like all spiritual, political and artistic configurations, is based on anthropological premises that are directly concerned with the worldview of an age – a worldview that some authors, many situated at the borders of their own age, consider worth pursuing for the sake of humankind as a whole.

The “global” scope of utopian visions gives rise to utopian plans that, if realized, could radically undermine the authors’ purposes, transforming the happy form of life they envisage into a totalitarian reality. Here we face the risk, run by many utopias, of creating dystopias.

Nevertheless, precisely through this approach, we find – within explicitly critical and satirical forms – a focus on the liberation of the weaker parts of society and an inclusive understanding of human problems. After all, political and spiritual movements – as well as many of the declarations on whose premises human rights are based, such as the Declaration of the Rights of Man and the Citizen (1789) and the Universal Declaration of Human Rights (1948) – are children of both the utopian spirit and the historical tragedies of their age.

Studying the universalistic focus of the history of utopia thus involves studying the many forms through which a society conceives of itself beyond itself, its boundaries and its weaknesses. In this sense, our globalized age is also a child of global, philosophical, political, spiritual and technological utopias.

One of the most recent fields of research in this area focuses on “globalization” as a key consequence of modernity. The last thirty years have witnessed a series of phenomena the rapidity and innovativeness of which are far from being completely understood: on the one hand, the incredible digital, technological acceleration of the past few decades and the transformation of the capitalistic economy; on the other hand, the apparent creation of a world characterized by forms of living and challenges that are increasingly common to the whole of humankind.

Globalization taken in itself and the global phenomena that characterize our contemporary civilization embody images from past global utopian endeavors while producing their own global utopian visions.

For this issue, we are seeking articles that examine this theme in general, and the following sub-themes in particular:

• forms and images of global utopia:
• contemporary global utopias;
• global utopias throughout modern utopian thought;
• globalization as utopia and dystopia;
• the forms and history of the global city.

We welcome submissions that take a philosophical, literary, artistic, theological or political approach and that are written in one of the journal’s five languages: English, Italian, Portuguese, French or Spanish.

In searching for a book cover image above I came across this one:

This Venn diagram is interesting in itself, but please reflect upon it across the 'range' of Hodges' model? What stands out to me is how corporations and health care still grapple with this question.

It appears they struggle to find the 'magic' - let alone multiply it.

Is Hodges' model a Self-Organising Hypothesis Network?

"The proposed approach is based on 3 key steps. First, derive knowledge from different sources of information (experimental data, expert learning, machine learning). Secondly, unify the different knowledge representations. Finally, organise the unified knowledge in a way that captures its generalisation hierarchy and facilitates the design of efficient prediction algorithms (Figure 3)." page 2 of 21.

Figure 3 The SOHN methodology. Different sources of knowledge are unified using a common representation based on the concept of hypothesis. The hypotheses can be organised into a hierarchical network to capture the knowledge in a standardised way (page 3 of 21).

Straight away this figure in Hanser et al. (2014) suggested learning and learners and the four care domains of Hodges' model. There are profound differences of course.

The information at the start takes the form of concepts, data, but 'information' also applies. The context in combining chemistry and informatics is radically different when it comes to who, or what is learning in that first arrow. The paper's keywords are:

Machine learning Knowledge discovery Data mining SAR QSAR SOHN Interpretable model Confidence metric and Hypothesis Network.

To a healthcare or social care student the knowledge encountered may well be raw. The principle of Hodges' model encourages the generation of hypotheses within and then across the model's knowledge domains. The learn-er can then attempt to unify these thoughts and reflections, organising them in a variety of ways. Hodges' model is not itself in self-organising, but there is a pattern-learning process for the learner and a pattern-matching process for the expert or specialist.

h2cm: what's in a name? enquiry to Barbara Rylko-Bauer

In October 2006 I exchanged e-mails with Barbara Rylko-Bauer who is Adjunct Associate Professor at Michigan State University; a medical anthropologist interested in health disparities, violence, human rights, and applied anthropology. Back then Barbara had questioned the use of the term 'career' in the title of Hodges' model.

Now with the website - purposes/audiences/content/strategy - review in mind I contacted her again - our correspondence edited for space and context is copied below.

On the website and W2tQ there is a post that references Hughes and the life chances meaning of career. I think Barbara's points about a subtitle, heuristics, synthesis, the knowledge (care) domain labels, e.g. sociology, h2cm as a pathway and other thoughts must certainly influence future developments: see what you think: ...

... I went back and looked at Hodges' model to refresh my memory...and I think that what struck me most was that it serves as both a conceptual tool that encourages a more holistic way of thinking about health and health care, and a heuristic tool that provides links to many different sources, some obviously about health, others less so but still relevant.

So, I'd first ask if there is a way of conveying that across in the title. You might need a subtitle: A tool for holistic approaches to health and health care.

I'd also suggest that your domains could be expanded to reflect better the broad spans of information they cover: so social or social/cultural (rather than sociology) and political-economic rather than just political. Just a thought.

As for the title, in skimming your explanations of "career" I think that a phrase that conveys the broadness of this concept, that it is all-encompassing rather than focusing on "career" in the conventional sense (although clearly the model evolved out of thinking about training of nurses). For example: human-health career and care domains -- one could use "patient" and yet, health and health care goes well beyond concerns of humans who happen to be patients.

Happy New Year!

Tx for sending me your chapter.... I was not able to open it but did skim what you attached in the e-mail, and it reinforces my sense that you should try and find a way of presenting the model as a tool for both finding and for synthesizing information on health and health care. I don't think that this is clear just from looking at the title of the model as it now stands or from the way it is imaged. It's really a pathway more than a model...does that make sense?

Go ahead and post my comments. I would prefer that my email address not get posted, just because I'm already swamped and would hate to get emails from people that I could not properly respond to (seems rude).
